Havent seen this mentioned here yet. Witch Hazel has made things almost bearable for me.
I notice when I dont use it I am in more pain and when I do it helps a lot. The idea is to keep the wound clean and the trick is not to aggravate it when cleaning, so finding a balance is key, I guess it works kinda like a sitz bath.
When I was having hard BMs and the pain was on a scale of 7-8 almost feeling like I was gonna pass out I eventually realised a way to reduce the pain, get a few tissues and apply hard pressure down there, I dont know how that works but it did for me although it was annoying to do it was better than the excruciating pain.

I used witch hazel some during my early fissure days. I used everything I could get my hands on, but one thing that really helped and you may want to try also was aloe. I got mine straight from the plant. I think at one time I almost healed the fissure in the beginning using it but my diet was fighting against me since I really didn't know the do's and don'ts of eating with a fissure.

Actually I just did some reading and putting alcohol on an open wound stops it from healing and the witch hazel contains alcohol so I have been working against myself this whole time, gonna switch to saline, hoping saline after a bm and half a sachet of movicol does the trick, I will look into aloe.

Use fresh Aloe Vera , and warm bidet and you will see immediate improvement
