Sentinel pile - is this painful??

Hi

I hope somone can answer this for me, I have had an anal fissure for nearly a year , had a few setbacks , but I have now got this pea sized hard painful lump just outside my bottom! it's not inside at all! is this a sentinel pile? Do they come and go? Do they flare up and become painful, different sort of pain feel very bruised and swollen down there, is this just another pain to put up with?

any answers would be greatly appreciated.

It could be a sentinel pile if it's where your fissure is. For most people, that is at the back, around 6 o'clock.
Sentinel piles can get irritated. Mine isn't coming and going, it's very much there all the time, and it's not painful.
But yours sounds like an external hemorrhoid. Your description is very like the external hemorrhoid that I had. A pea-sized lump, hard and painful just exactly describes it. It was to the side of my anus, not near the fissure.
Mine lasted about a month and then vanished.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

Thanks for the reply, I hope it just goes away then, is there any thing I can do to help this go away, and help the pain ?

There are hemorrhoid ointments, like preparation H, but it's not clear how they interact with fissure medications.
In fact, some docs treat hemmies with eg nifedipine.
You could ask your doc for lidocaine or lidocaine/prilocaine for pain relief. It didn't help me at all, but you never know. Ask for a free sample!
