Lord's Dilation procedure

Postby Shantanu » 25 Dec 2013, 01:41

Hello everybody and merry christmas !

My case is a special one. I have had two surgeries 3 days ago, one for a Pilonidal cyst and the other for anal fissure.
The anal fissure was chronic and I had a Lord's dilation surgery.
Pain was about level 4 on day one after surgery, with mild fecal soiling and bleeding.
The pain has reduced by now, its about 2. What worries me is that I am having burning sensation and considerable bleeding on passing stools which stops after the BM. The BM is pretty painless, but bleeding scares me. Also i get a slight burning sensation on contracting the stomach after the BM.
Is bleeding normal for a few days? Or should i be worried?
Please help!!!

Hi there, and happy holidays to you too!

Its still early days, so you shouldnt worry too much. With other procedures down there, it usually takes a week or two for things to settle down, and I would assume its the same with a stretch.

What country are you in? I thought a Lords dilation was outdated and replaced by LIS.

Best of luck with your healing!

Thank you for the reply.
I am in India, and both surgeons I consulted suggested dilation so I presume it's pretty common here.
What worries me is that I neither had that bleeding nor that burning while passing stools before surgery because of the dietary changes and exercise, but with the nature of the fissure being chronic, surgery was my only option to a permanent

I guess LIS hasnt reached same popularity in India as elsewhere then.

Anyway, Im pretty sure the dilation (like other procedures) can make the fissure worse initially, but after a while the healing will kick in as the muscle is less tense, and blood can flow freely. Maybe it simply retore, or maybe they cauterized the fissure to make it acute again. Regardless, you now have a greater chance of healing. Just give it some time and make sure to keep the stools soft.

Thanks a lot!
Today's BM was pleasant to say the least, medium consistency stools, very little blood on TP
I occasionally grt a spasmic contraction of the anus, but it's reducing considerably with every day.
Will keep updating

Its only been a week, and you already have no pain at all. Wow. Thats great :-)

Usually when we heal, the pain becomes less and less, to be replaced by irritation/itching, and then to fade away completly. Im sure what you are experiencing are just healing doing its work. Dont worry about it, and give it some time, its still very early days, and you already made great progress.

Yup I take two 5 mg Bisacodyl plus 2 tsp. of Cremaffin..lots of fibre in my diet, leafy greens, oats, wheat bread, 2-3 litres of water, fruits.Avoid coffee.
I am having solid stools though, maybe due to the lack of physical activity.

I was advised by my doc to continue Bisacodyl for 15 days..10 more days i.e.
What's your take on that?
And should i take Cremaffin after that for a month, as I will not be exercising atleast for 2 more months due to my BIG pilonidal surgery.
Thanks for the continued support Mod! :)

Hmm, Im not a doctor, but I would rather take a good stool softener like movicol/miralax than Bisacodyl.
As I understand it, cremaffin is the same as milk of magnesia, and many in here take it for as long as it takes to be fissurefree. Even now after I healed up, I still take magnesium supplements to prevent retears, and I have no intention to stop any time soon.

You should also make sure you are getting the right fibers (insoluble), as soluble can bulk up your stool.

Ok I will keep that in mind..but then should I stick to the Bisacodyl routine for two weeks and then discontinue?
