Hi
I've had a chronic fissure on and off for 5 years.
My surgeon has recommended an advancement flap and botox injections. I believe that the flap surgery is similar to a fissurectomy but they do a skin graft to fill the gap left after the fissurectomy.
My only issue is that looking round the internet I can only find horror stories about how painful this procedure is. Has anyone had this and can let me know their experience good and bad.

Hey, thought I'd give your thread a bump so more people see it. I had Botox and that didn't hurt much at all so that will be a walk in the park.
Someone else was asking about an advancement flap up in the General section of this board, I believe, you should still be able to find the thread. I don't think anything about this surgery so I can't help there, sorry...
Did you have other surgeries to try to treat the fissure already? Or are you jumping straight to the flap?

I am sorry that we do not have any active members who actually had it , but I remember one or two :roll: .
It is very fine and delicate procedure and not many CRSs are willing to do it since they do not feel confident enough. If your CRS wants to do it - he must be very skilled !!!
Members who had it needed about a week longer to heal than simple LIS but pain was not much stronger than fissurectomy. If you ever had acute fissure pain when you needed to clench your teeth due to pain during a BM, than you can be assured that nothing can top that pain , except maybe pain after the hemorrhoid removal :roll: .
All in all - if you want to proceed with the procedure, pain level will be high for the first couple of days but should be controlled nicely with pain meds . Botox you will not even feel since you will be asleep when they administer it during surgery.
My only concern is that I never heard of that combo - they usually do LIS + flap , but maybe your CRS had great success this way ! Ask him how many patients he treated THIS way and what was his success rate (% of healed patients) - it is absolutely normal to ask that your CRS and you should !

Thanks for you comments. This would be my first surgery. I have had botox before though and it was not more painful than a fissure.
My surgeon is not keen on LIS although reading info on the internet it has helped a lot of people. I have to admit that I am slightly nervous of LIS because of the incontinance risk (although small). My job involves having meetings with clients and if I could control not gas (or worse) it would pretty much destroy my confidence.
The surgeon will examine me under anaethestic and may just administer botox. He is the only surgeon who does this procedure in my area so as you say it is quite specialised. I will ask him the questions that you suggested.
I am in the UK and having this on the NHS so you don't get to have a long dialog with the surgeon. In fact I haven't even met him yet just one of his underlings!

Hi Elf, I'm having an examination under anaesthetic too (NHS), according to my CRS he'll perform LIS if my fissure is still bad, typically though, at this time my fissure seems to be dormant, I reckon it has a mind of it's own...

Hi
Strangely enough after 4 months of constant bleeding and pain my fissure healed itself prior to my operation (which I had yesterday).
As you say they seem to have some intelligence.
I may post a diary of my experience as so far it isn't as bad as the horror stories that I found via google before i had the op.

Yes- isn't it interesting how people tend to post only horror stories and everybody who had great success and is completely healed never posts anything Image
It does great disservice to present sufferers since they get completely skewed view Image in negative way : (((
