i have a long history of surgery in the perianal area.
Two abscesses removed, though the second one caused a fistula which 4 years and a dozen doctors not knowing why I wasn't healing. Eventually I made them refer me to a specialist and ended up having the fistula laid open and a seton inserted.
I'm now on the third surgery and hopefully the last.
I remember the rash last time, but this time it's even worse. Last time I was wearing sanitary pads to try and keep the area clean, but they seem to worsen the rash. It gets red raw and hot in a perfect two semicircles on each cheek.

Any advice that people have would be amazing. I'm already doing sits baths daily. I've tried various creams which can be messy and coconut oil, which seems to be quite effective. Sadly I work on my feet and walk a lot in my commute, so the chaffing is oppressive.

Zinc oxide cream like calmoseptine can be helpful when skin is irritated. Calmoseptine has menthol too which I find helps with pain.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy
