It is normal to still have painful BMs, but it will get better and better as time goes on. Softeners are a must - and you have to drink lots with them too. Give the miralax a chance to work - it should help a lot, as once things are softer, they won't hurt so much. Hang in there!
They are individual sachets that you add to the correct amount of water. They take a few days to work, and you can adjust the dose up or down as needed. Remember to drink loads if you take softeners!
Movicol is much much better than lactulose! Just ask in the chemist for it. Boots will have it. Ask for lemon and lime flavout though - original is disgusting!! It's about Ј10 but you can get it on prescription.

It does sound like PFD, but on saying that, my fissure caused the same pain that you describe. When I got those spasms the whole area tightened up. LIS did cure that! And it is only a small op. If you had it done and it cured the anal spasm, it might only cure that but it might cure all of it!
