Some things that have helped me a little

1. Miralax.
2. Lots of fruit and veggies...especially pears and watermelon...but be careful not to overdue them so that you don't have too many BMs.
3. Sitz Bath after BMs. Sometimes it hurts too much to sit on the sitz too long and I'll get up and change the H20 a few times so that i don't stretch the area.
4. Non-strenuous walks after BMs...gets the blood flow going.
5. Healthy Fats...avocados, olives, olive oil...
6. Beans...see #2.
7. I don't sit down after a BM until it feels less sore...sometimes I don't sit for hours.
8. I am using ichthammol ointment (a "drawing salve" for boils, splinters, etc.) right now and it might be working. It is black and tar-like but it hasn't burned and it is soothing. I say might because I am also barely eating and have had small, soft BMs lately.
9. Heavy lifting or snow shoveling makes me a bit sore...as does stress. I think maybe I clench during those activities.
10.I gave up cheese, sugar and white flour :(

Interesting about the icthammol ointment... I know the stuff from childhood cuts and splinters. It's messy stuff like you said (tar-like), but good against infection and maybe good for healing.

I am cautiously optimistic about the ichthammol...I feel like the fissure is healing, but I have been here before. I am surprised that no one else has brought up the ichthammol because my GI doc told me about it. He said to apply it after BM & a Sitz bath.

I have no idea, but my totally uneducated guess would be that they don't suggest it due to its sticky-ness and also that there are other ointments out there that also have a pain relieve associated with them, or are targeted to increase the blood flow. When I was a kid, we used icthammol ointment like a general ointment... But who knows?? If it works, use it! Just like the Manuka Honey!
