Need advice for butt problems

Hey everyone,

So I’ve had hemorrhoids since I was 17 (now 33) I’ve had them popped, cut open, had an external hemorrhoidectomy this past July followed by an anesthetic surgery to remove Polyps. On top of constantly having those issues I have a somewhat large skin tag that usually sits externally but sometimes
Goes back inside. I have managed my constipation by changing my diet and using mirilax daily. No matter how soft my stool is I always seem to have pain. Like burning, stinging, sometime bleeding, and then it stays sore throughout the day.

I find myself having to clean around my skin tag multiple times throughout the day to remove fecal matter.

I’m at my proctologist regularly. He’s great but I really can’t live like this forever.

I take sits baths multiple
Times a day and use vitamin e oil inside and out because it feels uncomfortable when my skin is dry. I also use calmospetine cream before and after bowl movements.

I’ve been in pain for so many years, I have no sex life and I’m getting married in a month. Any advice would be appreciated.

I have a sensitive bum as well and what helped me for a while was gabapentin pills. Eventually I had to stop taking them because they really irritated my gut. I've found that a magnesium supplement helped reduce the nerve sensitivity too. My suggestion would be to apply fewer things to the area because sometimes too many ointments can make sensitivity increase.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy
