Is my anus too tight?

Hey I'm new here but I'm not new to anal fissures.
I've been suffering from them going on year now but it feels like forever!
I have Crohns in my lower intestines and have been newly diagnosed with perianal now Image & it all started with having bad constipation one time and it just open the gates of anal pain hell.
Either way my GI/surgeon told me I have multiple fissures & they're all very deep. I can feel three right now just laying on my stomach typing. I'm pretty sure it's like 5 there though along with a skin tag.
I've been relaxing, sitz bathing, I'm on Remicade, doing yoga, putting on aloe vera or/with lidocane (sp?) and it's not doing much it just makes it feel tingly and wet. The only time I felt normal was when I was re-upped with Remicade (given a triple dose) but they just came back when I ate something that didn't agree with me once.
I just want the pain to go away! I've tried the flagyll (sp?) and ciproflaxim (sp! I know I messed that all up). but they've done literally nothing. I think I have spasms (where it feels like it's tightening and relaxing randomly) and they're very bad. What can I possibly do?
Is my anus too tight because it always feels like I'm passing the largest stools and come to find out it's like toothpaste. Help!

Hey stopthepain--
My unprofessional opinion is that yes, it's too tight. :(
The way you describe it reminds me of what I went through (minus the Crohn's). I'm not around here much anymore but I know in the past there have been some members with Crohn's. It is still possible to have the surgery even with Crohn's, and if you aren't healing I would ask to see a CRS.

it's really not an easy question to answer in the presence of Crohn's disease, especially if your disease is known to be in the perianal area.. It could be really helpful in your case to have anal manometry performed to check anal tone.
Surgeons have differing opinions on LIS in Crohn's patients. Crohn's doesn't by itself rule the possibility of surgery completely out, but a very careful abd thorough evaluation is needed by a colorectal surgeon who is very familar with Crohn's and fissures, and particularly perianal Crohn's. Surgery can be riskier in the presence of active Cohn's and can carry with it more risk of complications, more difficulty with healing, and less chances of success. Again, it doesn't mean it can't be performed or that it can't be successful, it just takes a much more experienced and skilled physician to evaluate your particular case. I would suggest a few different visits to a few different colorectal specialists who can communicate with your gastroenterologist in determinijg the best possible plan for you.

My feeling is that anal pain, fissures and the like are all initially caused by "tight ass syndrome" - the muscles down there are too tight and so when you go, there is a lot of trauma. Stress can cause it, thats how I got in this trouble. You must relieve stress in your life, learn to meditate and RELAX, eat well....
