You would know more after consulting with a professional for sure, but their recommendation would likely be based on how you responded to non surgical treatments over a period of time. If the pain wasn't too bad to make that unreasonable.
I have internal ones and though it's been debilitating on some days, I do seem to be responding to treatment. Over the past month I've literally been knocked on my ass by my ass and can't work, but I have hope.

botox made my bm's easier but i had leaked a bit and had to run to the toilet as soon as i needed to go....really for me botox wasnt good for me..im celiac so lacking many nutrients speicialy magnesium wich relaxes the anal muscles and i have a yeast problem so have to get on top of it all..things for people to think about if they cant seem to heal or heal only to keep getting them back...always underlying issues to them coming back alot.

Deep as in really far inside or deep as in the tear is deep (like into the muscle)? I wonder about this as well. Mine is internal, but I can see the small tag and beginning of the fissure if I'm relaxed enough. It looks a bit deep to me. I can't see the muscle (at least, I don't think I do... I thought the muscle would be a white color, but I'm not certain). I'm just concerned if mine will heal by itself (i.e., without surgery) if it's at a certain depth and size.
January 2013 - Diagnosed with fissure. Eventually turned chronic.
History of IBS and anxiety disorder, along with fear of using bathrooms other than my own caused it.
Tried Diltiazem, but eventually developed a rash.
LIS surgery scheduled August 26th.
