AF Help. PLEASE?!

Hi i would like to thank everyone for talking about their own concerns. If it wasnt for this site id probably be still scratching my head wondering what to do. Thank you all.
Okay starting from the begining, I had blood during bowl movement, and pain during and after bowl movement. pain was horrible gut turning couldnt sit still after bowl movement kinda pain. I thought it was hemorrhoids, went to ER they said it was either hemorrhoids or an AF. I went with the hemorrhoids option. I knew what they were and figured I couldnt get AF. The suppositories that they perscribed were helping with the pain so I figured thats what is was. I ran out of the perscripiton drug and pain came back. I went to an urgent care to get another perscription, they then informed me that it was an AF. The urgent care doctor reffered me to a specialist.
Fast forward a week, had an appointment with my primary care doctor, he then told me that I had an acute condtion. If i increased my fiber and water. watched what I ate and took stool softeners, i would be healed or close to healed. No need for surgery. I told him about my appointment with the specialist. he told me that it wouldnt hurt to see what he said.
Two weeks between the primary care doctors and the specialist, I increased my fiber, doubled my water. Stopped drinking caffine almost all the way, (one every two days or so) take stool softeners twice daily (moring and night). watched what i ate. Stopped taking the suppositories i had that were helping with the pain. So the pain eventually went from a 9, 10 on a scale of 1-10. to a 1,2. blood stopped with the occational blood every now and then.
Day of the specialist appointment(today), specialist told me that i had gone chronic and need to go under surgury?
So what I am having questions about is, to my understanding if it was now chronic wouldnt there be just as much blood and pain as when i first got it? and how do I go from acute to chronic in two weeks when the symptoms say I am on recovery. the spasms before would literally not let me sit still at all with out feeling like i had to throw up. Now i can sit stand do whatever, yes i do have a spasm every so often but they are not as bad and last no where near as long. Does this make any sense? I just dont understand, I followed what my primary care doctor told me to do, started getting better (so i thought) to going to chronic? Should i just go thru with the surgery and deal or see if it gets better on its own? I am just so lost

I guess what im trying to ask is,
What are the symptoms of a chronic fissure?
What would you do in my situation?(having two completely different opinions)
Does is sound like im getting better to you?

If your pain and bleeding are improving with your current regimen it would definitely be worth getting at least one other CRS opinion before consenting to surgery. Fissures aren't generally called chronic until they have been present for at least six weeks without improvement/healing. If it were me I would want at least one more opinion by another specialist.
It is possible in some cases that the chronic fissure isn't as painful as it is in the acute phase, particularly the pain associated with bowel movements, but I would still want another opinion before considering surgery.

Thank you I was looking into getting another opinion soon. I just have a lot of questions and the doctors seem to not want to answer me.

Yes, this can happen with doctors sometimes and we all have to stand our ground to get our questions answered and to feel comfortable in our treatment plan. It isn't easy at times, particularly with fissure issues because it leaves us feeling vulnerable and we are in so much pain. It is helpful if you have someone who can go with you and help be an advocate for you with the doctor. Hold fast though. You deserve answers, and to understand what your condition is and to be fully informed in moving forward.

Definitely get a second opinion, it literally cannot hurt, and will definitely hurt less to get another opinion then under-go surgery.
Surgery no matter how minor is Surgery, not to say that it can't be relatively safe under the care of a competent physician. But if it can be avoided, your better off for it.
Also specialists will definitely know better then G.P.'s, as specialist deal with the same thing on a daily basis. General Practitioners, are extremely knowledgeable and necessary, and are required to understand a bit of everything so obviously their expertise can only go so far in any specific case. I agree with the previous comments, consult with another specialist.
