AF - What reports to take to a doc?

So I am planing to see a GI next week. So far, I've been to my general practitioner, 2 CRS'. Just to eliminate anything more sinister, and to have a more informed opinion on my constipation (rather than self treating it), I am planning to see a GI. The GI's office said that I should bring in any reports that I have on my condition. I got an anoscopy done at one CR and sigmoidoscopy done at the other one.
Are there any reports for anoscopy and sigmoidoscopy? Is there any general report which a CR prepares on the AF, which I should ask for?
Any help here will be appreciated.

The anuscopy probably doesn't have much of a report. The doctor probably made some basic notes about it in his progress notes, but I wouldn't expect much more than that.
A sigmoidoscopy report may have a bit more to it, although even that may not have a full blown report the way a colonoscopy or MRI/CT does.
I would just ask each of the doctors you've seen about your AF for a copy of the medical records including any procedure reports and progress notes. The progress notes should have relevant info for sure. You'll usually just have to sign a privacy release. Doctors' offices are required by law to give you a copy of your medical records, although the fees they're allowed to charge and time they have to comply varies by state. At most clinics, the front desk will just make you a copy on the spot for free, especially if it's a specialist that only has small set of records for you (as opposed to your PCP who may have much more comprehensive records).
