Anal Advancement Flap...

I have read accounts of people who described having had this done, on this board I believe....
In some cases the risk of incontinence is too great to justify doing lateral internal sphincterotomy. This may be true for women who develop a fissure while giving birth, because they typically do not have a high resting pressure in their internal sphincter. A procedure called anal advancement flap may be done instead of sphincterotomy. 1 In this procedure, the edges of the fissure are removed, and healthy tissue is sewn over the area.

i think if you are going to do that, you may as well get a whole brand new one!
hehehehehe......
i don't know but it sounds like a fistula waiting to happen or another fissure. i can't imagine that the anus would have enough elasticity if the new healthy tissue was sewn over the 'fissurectomy' area......is seems like it would make it tighter, no?

An advancement flap is kinda like a skin graft. A square-ish piece of skin is cut out, basically the fissure and then some, and a piece of skin from above the site of the fissure is pulled down over the newly opened square and fit in kinda like a puzzle piece. The edges are then sutured shut. The primary wound (i.e. where the edges of the skin meet) should heal in about 24 hours, so if you can keep from pooing for that long or longer, you would be in pretty good shape probably. You will still have to deal with the fragile skin, the rest of the wound healing, basically the problems you encounter from dealing with any area that has had stitches. However, there is always the risk of the stitches pulling, becoming infected...
Advancement flaps are a pretty comon way of dealing with non-healing wounds. Diseased tissue is cut away, the wound debrided, and healthy tissure sewn over the open area.
Also, it is usually used for people who would be put at risk by an LIS, including elderly people and the women Kim mentioned.

MK - it is very good procedure if done by good CRS !!! It can give fantastic results for people who tried all else and it failed (like LIS and fissurectomy and such).
We had one lady here who did it and had great result !!!! The procedure is not favored because it is very "finicky" in the sense that it requires a surgeon that is REALLY good and experienced with that kind or almost "small reconstructive" surgery. Also - sometimes a day in a hospital is needed and it is expensive - so most doctors just "skip" it as option. If your surgeon had great results with it - you really have no more risk of developing any complications after the flap than you would after any other butt surgery !!!! I wish you the best of luck with whatever you decide to do : ))) !!!

this procedure fixed my 2 severe fissures after 15 long years of suffering...hallelujah...do it, do it, do it but expect a long painful recovery...well worth it

Hi, this procedure was recommended for me because of scarring and the inability of the area to stretch when I have a bm. When you say a long and painful recovery what exactly are you talking about. How long and how painful. Please be specific. Thanks Suzy
