I need a list

Postby Guest » 16 Nov 2007, 19:17

Hello,
I have been dealing with a HORRIBLE fissure for about 4 months. Terrible, terrible pain.
My family doctor was no help and told me to get some hemroid ointment!!!
I have an appointment on Monday with a new family doctor who I am going to ask refer me to a CRS. I know it may take a long time to see a CRS so can anyone please recommend me some medications I could request from my family doctor to help heal and alleviate some of the pain I am having. Thank you.

Yes, where are you located ? If you are in the USA, you should be able to see CRS very soon too!!! In the meantime, have very warm baths 3-4 times a day for about 20 min. If it helps, you can buy heatpad and sit on it while working, watching tv etc. Try Ibuprofen for pain ot Tylenol . Ibuprofen is much better, but I couldn't use it because of my stomach. Make your BMs ulra soft with metamucil, mineral oil or milk of magnesia (just a tsp ) twice a day. Put something oily on you anus before "you go" so it will make your skin more lubricated. Eat only cooked food -mostly vegetables and fruits, less meat and milk products - skip those if possible completely.
BUT, you must see CRS as soon as possible - nobody else will be able to help you !
Best of luck and I hope something of the above will help !

Fissulyna's question is pertinent: Where you live may determine what is available.
I know in the US, one can "generally" request nefedipine or ditiazem (both will be compounded at 2% with lidocaine) which are blood pressure meds that will lower the pressure inside the anal canal and give you substantial relief from the pain, and some people will heal with just these creams. Or nitroglycerin, with also can help quiet the sphincter (but can cause severe headaches as a side effect.
You can also request that the referral is made quickly; that may or may not speed up seeing your CRS. There is no reason why your GP can't prescribe the creams, unless he/she simply is unaware of them as an AF treatment (too many are) and refuses.
In the meantime, if at all possible take 3-4 hot soaking baths each day to provide both relief and healing. If done religiously, this small step can help lower the overall pain levels, but it generally requires more than hot baths to really bring the fissure pain under control.
Stop eating all junk foods, and less meat and dairy. Right now, fruits and veggies are what you need, and be sure the veggies are cooked. Stick with soft foods as much as possible and avoid hard, crunchy items that may bot digest and can scrape past the fissure, causing more pain.
Get your recommended amount of fiber and water each day, and take a stool softener like colace or MOM.

The first and most important thing I would recommend is to get 25-30g fiber a day, every day, with no exceptions. I have never taken stool softeners simply because I have a very high-fiber diet. Eat a cup of All-Bran every morning and eat fiber-rich foods throughout the day. When you eat your All-Bran or anything very high in fiber, drink a big glass of water during and after you eat it. This is super important otherwise you don't get the full effect of the bran. In fact drink 8-10 8oz glasses a day, this will also keep your stool soft. You can google "fiber" to find out what has fiber in it, but make sure you get both insoluble and soluble fiber. Good things are vegetables, beans and lentils, whole grains, etc. Fiber will soften your stool and make it much less painful. It is crucial that you avoid constipation at all costs, and avoid holding it in no matter how painful it is.
AVOID eating the following: anything spicy, acidic (ie. tomatoes, citrus fruit, etc), alcohol. Decrease your intake of dairy (esp. cheese) and red meat. Chew your food well.
After you have a bowel movement, hop in a warm bath right away. This will ease pain a great deal. During the BM don't strain, it will rip the fissure even more.
Pain medications like Tylenol or ibuprofen are indeed very tempting when in pain but please don't over-consume, as they are constipating for most people.
I hope you find a surgeon soon and don't despair!! If you have any questions at all, please ask us, we're all really open-minded here :)

I just wanted to clarify that constipation is not typically a side effect of Tylenol and Ibuprofen. If you add a narcotic on top of it it certainly can be though.

If the pain is too intense you might ask your doctor about Ultram (tramadol). It is non narcotic and though it can be constipating it usually isn't as bad as a narcotic. They even have an extended release version too.

Yep-ultram has been around for many years. I don't know about the extended release version but tramadol (the generic name) should be available in Canada.
