No pain when poo, knife feeling all day from time to time.

Hello,

I have an anal fissure. Two proctologists examined me. Clean colonoscopy. I had blood a time or two at the beginning. Nothing hurt. I started using dialtezam and suppositories. After some time I started to sting, sharp pains during the day from time to time. They are not very strong. I wonder if my condition is getting worse or is it normal. I do not, and have never had, pain when visiting the toilet. Does it bode well or should I worry?


My symptoms:
*no pain when defecation
*sharp pain from time to time all day but not very strong
*no bleeding (only once or twice)

Will it be worse?

I think as long as there is sharp pain and spasms it's most likely a fissure. Have the doctors told you when to follow up with them to check that things are healing?
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy

I go regularly. But you think it will became worse?

Should I use pranicura instead of diltiazem?

I honestly cannot say if you will heal or not. Just make sure to keep bowel movements softer and regular. As for medication, it's probably better to use the prescription ones, but if pranicura doesn't have anything that will interact with diltiazem then you could alternate them during the day. Just remember not to smear on too much ointment because that may lead to rash from persistent moisture.b

I use pretty much diltiazem now... is it bad?

Not at all. I think it's clinically proven to heal some fissures.

Statistics says 70% chronic fissures are healed by diltiazem

I hope some of my experiences can help answer your question. I’m not a doctor and can only pull from my experiences, but I had the same symptom and question back when I first joined this site.

I felt exactly what you described; pain throughout the day but not when I actually poo. I’ve also had the more the common “passing glass as I poo” pain too. Another source of pain for me during the day (as in not when pooing) is pelvic floor muscle spasms caused by the fissure and also by stress/tension issues. Healing takes time and there may unfortunately be steps backwards along the way. I wish I could give you a timeline but some fissures heal fast and some take weeks or months to heal. Finding a diet that produces soft, formed stools and adopting various stress reduction techniques were what ultimately helped me most. I also spent a lot of time reading posts on this site for continued reassurance. I wanted to answer you because I needed to hear someone tell me they felt the same pain I was experiencing.

That said, my doctor told me that she never had a patient whose fissure didn’t heal and could never give a firm timeline because everyone is different.
