I second what jr2 said, all :). They are even doing dilation at Cleveland Clinic now, albeit the balloon type. I am preserving. Mostly stopped my bleeding (using 11mm), and decreased spasms. Like jr2, surgery is not an option for me.

so, for more than a year I had anal pain just once. It was my fault. I inserted the big ball (on the "Groove") too fast, without good stimulation and relaxation. It took one week to cure.
As I wrote, I started with http://gadgets.boingboing.net/filesroot ... 00x200.jpg it relaxed my sphincter and I realized that this is very interesting idea how to cure anal fissure.
Than I bought http://www.amazon.com/Platinum-Pure-Silicone-Groove-Blue/dp/B00A8VHATK It has very narrow end and really good for the start.
Now I'm using http://www.amazon.com/Exotic-SILICONE-LOVE-RIDER-KISS/dp/B00E8R2HJI for fun and to keep my sphincter "in shape"

Use lube and give it some time, do not use big diameters when your sphincter is not relaxed enough.
To avoid constipations I eat yoghurt.

shit, shit, shit!!! it's back again... after a year of no troubles..
I think I did too much work at home which is required lots of muscle strength

a quick fix:
- fibre supplement with every meal (+ water)
- wash it inside and outside after the toilet
- apply nitroglycerin ointment before go to sleep

Good luck, VVV, I hope you heal quickly. Your body just had to remind you that you need to be careful! At least you know what to do to get better. Hopefully you'll go a long time without any more pain.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

bloody hell... still fighting. It's not too bad (I'd say on the scale 2 out of 5) but I can't fix it this time for 5 months already.
Lots of fibre but more water I drink- I just more urinating.... weird.

Are you using any of the fissure ointments?

I'm using Rectogesic and sometimes coconut oil. I'm sure there is something wrong with my digestive system because poo is not soft enough. So, sometimes I use enema to inject some water to soften it.

I think the vibrator makes complete sense! I am going to try it... my fissure is very close to the opening though so I may not use the vibration internally. I am not very knowledgable about sex toys, but I'm sure you can find a small vibrator, about a finger size, right?
