had bad itching/pain this afternoon after sitting for a while, led on my back when i got home and looked in the mirror, my hemmis look more inflamed and sore. they are obviously getting worse!

When I have my EUA should I ask for her to remove them? (they are small external ones but obviously hurting) what are my options? already had sclerotherapy which failed.

please see my photos attached to this post - sorry they are so graphic but we've all seen it before and I really need some support right now.

Oooo sweetie..... They look sore.
They look like ' new' ones if you know what I mean. Inflamed and purple. I can see why you are having so much pain and discomfort.
The good news is that they don't appear to be very big, so hopefully they will subside in a few weeks with some cream and maybe cold packs??
This is just my opinion though...... I'm no expert.... But you are asking for more trouble having them cut off.

To be honest they've been there for around 4 months now but only recently been getting worse. Not sure what to do!

I did not look at the pics! Having had really nasty ones from time to time, I know just how bad they can be. And surgery... If I could turn back the clock would NEVER have had surgery. I go with what OMB said, that is that you invite more problems (scar tissue, etc). But, you are the one suffering so so it's up to you in the end. Sorry about the seeming pun!

Ow, that looks nasty and painful. I can't imagine surgery would help, and might take forever to heal. Ice sounds like a great idea. Lidocaine for the pain? Wish I could help more.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

Thanks for your help guys. One of them had thrombosed sadly on the right side I can feel a noticeable hard lump under the skin right in the sphincter. When I push on it it's really painful :-( what are my options do you think?

If it's thrombosed, the pain should be excruiating. Had one of those and had emergency surgery. Have a look at this article. Mine was about four times the size. Years ago now. You must decide - hard as it is - for yourself as to whether you think yours absolutely merits surgery:
