Doctor Says I'm Healed...Discomfort Persists

Hello,

So, after 6 weeks of using Nifedipine (4 times a day, 3 weeks - 2 times a day, 3 weeks) I went for another check up with the CRS who says that he no longer sees any signs of a fissure and to stop the Nifedipine all together and "have a nice summer". Meanwhile, I am still experiencing discomfort and an ache. Also, I am feeling the need to urinate pretty frequently which seems to be tied into this fissure.

Is it possible for the pain and discomfort to continue even after it has healed? Has anyone had any bladder related issues such as feeling the need to go pretty frequently and a dull ache in the penis which feels like is is radiating from the anus?

I'm feeling very confused...

Yes to all of your questions. There are a lot of nerves in the anus and I guess some of them goes to the penis. I had some penis/bladder symptoms too.

Its still very early days for you, so give it time. Your fissure might be healed enough to be invisible from outside, but it takes a long time for it to be completly healed up, and for the tissue to regain its strength. Usually it takes months for all the posthealing discomfort to settle down.

Thank you so much for responding. It is comforting to know that this is normal.

I am tempted to continue using the Nifedipine as it makes me feel less symptoms.

How long did you use Nifedipine?

Well. I used rectogesic for 2-3 months, then diltiazem, and then I had botox. I continued to use diltiazem a few months after botox.

If you do not get any allergic symptoms (rash) by using Nifedipine, then dont worry about staying on it for a while after healing.

Sorry to hear you still have pain. JHH is right in that is seems to take quite some time for things to settle down. The nervous system has to unlearn its oversensitivity.

I asked my CRS about this very thing yesterday and he said it was safe to use the ointment indefinitely. Many take it as a blood pressure medication, long term, so it should not be an issue (unless it irritates the skin I can see no reason to stop if it makes you feel better).

I would def. recommend continuing with the nifedipine for as long as it takes. Also, remember you are still fragile down there, and exertion can set you back. Take it easy. Don't do too much bending or lifting, and listen to your body. If you start to get more achy, slow down.
You'll probably have some pain and discomfort for quite a while, but it should improve with time. Just don't get discouraged if it seems to take forever, you'll get there.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

Thanks a lot, I really appreciate the responses. It's definitely unsettling when a doctor tells you that essentially you are cured and meanwhile it seems like I still have many of the exact same problems...although I know that they are much better than when this started.

I guess the bladder/penis issues give me anxiety because I get worried that this is related to something else. But I have been checked for EVERYTHING. No bacteria, no prostatitis, stomach is fine according to the Gastroenterologist (other than a little sensitivity but probably just because I have been taking Metamucil). So, I guess it has to be something radiating from the fissure. I guess everyone's symptoms are different.

Msimon,

Thank you for this. It is something I thought about but it doesn't appear as though I have the majority of the symptoms that people with this condition have. and luckily my symptoms have improved for the most part as this fissure has healed. But there are some symptoms related to a pelvic floor disorder that I do have and don't want to ignore.
This book seems to be a sales pitch for their clinic...are there any resources that explain how to actually deal with this?

Yes, it does come across that way. There is actually a lot of useful information in the book though. It explains the causes of pelvic pain quite well and also instructs on how to do trigger point release and breathing techniques. It is very useful if you don't have a lot of understanding of the muscles down there as well as the how the pain from muscles can refer to other areas. It speaks at length about chronic prostatits so I thought it may be helpful to you. Have you read the book? Many have gotten it from their local library so it didn't cost anything.
