Hi guys,I really need some advice on this...I had a fissure for two months and it healed with nifedipine; after 6 weeks of being on the medication, I went back to the doctor because I was still having pain after bowel movements; dull rectal pain throughout the day but I was surprised to know that my fissure had healed. I've been to 4 doctors (2 general surgeons/ 2 CRS) and they all have stated that they don't see an active fissure and have referred me to a pelvic floor therapist; it has now been 3 sessions but it only provides temporarily relief just like like sitz baths. After being on the nifedipine for 6 weeks, I felt as though I got an allergic reaction to it so i stopped using it, I felt rectal burning/ stinging, lesions (paper cut feeling around my anus) so i started using A & D ointment, vaseline, coconut oil, vitamin e oil, tucks instead but realized that I only get temporary relief for a day and then the symptoms continue. When I have a bowel movement (without lubrication its worst) it stings and burns and sometimes I see a little blood (some bowel movements are painful and some are not); BM make the pain worst and other days it reliefs pain, my anus hurts (burning, irritation) when I sit for a long period of time; I feel dull ache (soreness)/ pain all day (pressure in the rectum as though I need to defecate and some days I don't). I have good and bad days in terms of pain ranging from 1-6 (stinging, burning), I feel as though my anus is tight but I have a normal sphincter according to the CRS. The odd thing is when I go in for an exam, I feel some pain during it but feel relaxed afterwards. My anus feels as though it is not stretchy, (paper cuts feeling around my anus like chapped lips), some days it is sensitive to the touch (inside and out). There is no visible irritation or redness (to me or doctors) but I feel as though I have it. When I don't add anything there it feels really dry and chapped. This week, I started to have painful bowel movements so I applied nifedipine; today is a good day but the constant ups and downs have me down. One day you feel as though you're getting better and then the other you're in pain again. I know I should not be complaining because there are others that are in much more pain than I am but I am desperate for answers. Any input would be appreciated...

Wow,your situation sounds EXACTLY like what I have gone through. Was fine one day, then I all of a sudden started getting this awful urinary urgency, inability to empty bladder, felt like urine was constantly about to drip from my penis. It was terrifying, had no idea what was going on.

Not sure if this was caused by a fissure, or, if a fissure formed as the result of this. But I did notice some anal pain so I went to a proctologist who diagnosed me with a fissure and gave me nifedipine. He also examined the prostate and confirmed that it was not inflamed so he doubted prostatitis. So, great news! I just have to cure this fissure and I will be fine!

Well it's been 3 months. The fissure is visibly healed, yet, I still have the same type of pelvic discomfort. I wouldn't call it pain so much as I would an constant discomfort/annoyance. Heels like I pulled a muscle in my pelvis. Feel it in the scrotum. Granted, I am not in the extreme situation I was In When this first started...but it is still there, I still feel it.

How are you feeling these days? Any more progress?

I've had some progress but I am not completely pain free. My case seems to be a difficult one! I was referred to pelvic floor physical therapy because my muscles (in the pelvis) were tight (happens when the body is trying to protect itself) and was diagnosed with pelvic floor dysfunction. I've had Botox done twice in the spasming muscles and it has helped decrease painful bowel movements, irritation, and the ache feeling around the rectum area. I am now dealing with irritated nerves (tight muscles irritate nerves) and even though my muscles are not as tight my nerves remain irritated so sitting causes me pain. If I can help you in any way please contact me. I would suggest that you ask to get referred to a pelvic pain specialist and a pelvic floor physical therapist for evaluation.

I too seem to be a difficult case. I am just about to start some more pelvic floor treatment with a different therapist. I have had fissures and loads of surgery, including two loads of Botox into sphincter. All this has gradually lead me to become hypertonic in the whole pelvic, muscle guarding I have been told. I am so scared that I will go backwards again although I know I have to try this as my life is not what it used to be. You will understand this, we develop and live by our coping strategies. How do you identify the nerve irritation? I am curious to know whether some of my pain (on right side of external sphincter mostly, and into inner buttock, is nerve pain. How are you at the moment? Any progress with your treatment?

The doctor named it neuropathic pain; the pelvic area starts hurting about 15-30 after I sit down.. Then I feel as though the area is burning. I no longer have the irritation thank god but now I am dealing with nerve pain which is so painful. Basically the pain is on autopilot (nerve pathways); the central nervous system has gone haywired. I have to meditate and relax to help the nervous system to calm down. It is a lot if work.

Alyssa, This is awful. Pain, especially for the time you have been suffering is so exhausting. There has got to be something to help you? I take it you have tried all the usual anti-depressant type nerve pills, nerve blocks (although I have read they don't always help). It sounds so complicated when it gets to the brain and nervous system. Has your pain steadily increased as nerves have grown back do you think or could it be an entrapment issue? There is just not enough known about this type of issue and seems that the professionals who should are not aware even though pelvic floor is linked to so many other areas, ie gynae, colorectal. So frustrating.

It is very frustrating; I've tried several medications but nothing helps they only make me feel like a zombie. I've tried nerve blocks as well; with no help. I have no entrapment.. The pain disappears as soon as I stand.

The pain has spread because it has been going on for almost 2 years in February. St this point I feel as though my body is all sensitive because my internal alarm system continues to fire although there is no harm. I have learned a lot about the nervous system within the last couple of months. There is nothing structurally, wrong; we can continue working on the painful areas with no results. We now have to aim all efforts to the brain which is the area(s) that is sending the painful sensations to the pelvic area.
