Help please. Anal fissure surgery

Hi all you lovely people.Sorry in advance for this super long poat but I really need some help. I am a 26 year old female and have a 6 month old daughter born through emergency c section. After my labour and all the pushing i developed internal piles and a chronic fissure that doesn't seem to heal. I have completely changed my diet and using MOM so stools are soft and regular but the fissure just doesn't want to leave me. I have a sentinel pile as well hanging out.
The pain was quite bad at start but now not so much. I d say about a 3/10 after a BM but subsides after a few hours pass by. I am not worried about the pain as much but what is depressing me is that this has just taken over my life. I am always thinking about my next BM. Always making sure i have 30g of fiber a day - no junk food etc. I can not enjoy my baby or even be romantic with my husband.
Already its so hard being a first time mom but this bum problem has been going on for so long now that I feel my life is over.
Anyway i have seen an NHS CRS and he said that since rectogesic didnt work surgery is the only option. He wont do LIS on me because he said you might get incontinence and that will be irrreversible. So what he is going to do with me is to cauterize the sentinel pile and scrape the fissure to make it fresh to encourage healing. He warned me of the pain post op. But also said that the success rate will only be 70% which is so disheartening. I dont mind the pain for a week or two but i just want it to heal. But a 70% success rate is not what I wanted to hear. I have already gone through one surgery 3 months postpartum where they removed a skin tag, gave me botox but missed the fissure. And the pain after that surgery was so awful that I ended up in A&E. So because I already know what an anal surgery recovery is like its making me even more scared. Plus I think I can only go through this one more time.
Please could anyone who has gone through the same sort of experience give me advice.
And are there any people here who have gotten rid of this fissure in the long run whether by surgery or self remedies? I just need to hear some successful stories to make me feel better that this is not the end of the world and I too will one day have the 'luxury' of pooping with zero pain.

Hi,
I really do feel for you I have had this a year now an quite frankly it's been the worse pain ever. I don't know how you are doing it with a baby.
I can't help with the diet as I got my fissure due to IBSD so I spend my time thinking of things to slow me down,
But have you thought of getting a second opinion, the first CRS I saw refused LIS straight away. So I paid for a private consultation he didn't recommend LIS straight away due to the length and depth of my fissure. But and this is the important bit he did say that if your resting sphincter pressure is 4 or 5 then the Incontinence is never more than marks on pants...now I'm in agony and was like look I will wear a blooming nappy rather than have these spasms. My pressure shockingly enough was a 5 and he has happily agreed to give me LIS after Botox to try and get some healing started.
He also very kindly transferred me to his NHS list and I got the Botox after 5 weeks. Which has been good but I'm still in pain, and will be going back in September to discuss LIS plans.
It was a lot to pay but I had a 40 min consultation instead of the by the book no LIS for you coz you have had children. I guess it's how much the pain and inconvenience is annoying and upsetting you as obviously there is always the chance of incontinence.
All I can say is I had the scrape with my Botox and I haven't healed but he did say once he saw my fissure it's one of the worst he has seen. But the recovery from the scraping wasn't as awful as I expected after 5 days it was just a bit painful and by day 10 I could walk normally again.
Fingers crossed you find something that works for you...ask to have your resting pressure tested

Hi mypoorbutt thank you so much for your reply. Yes I am thinking of getting a private consultation done because I really can not go through this again if I am not sure this will be any more successful. Plus the CRS I saw really did went by the book and just saw me for a total of 5 minutes. I had a list of questions on my phone and he was literally holding the door while I was trying to finish off the list of my questions. It was so embarrassing. The waiting list is just awful with NHS. Is the scraping same as a fissurectomy? What was the pain level in the first week post op and was it under GA? Hope your LIS is a success. Will be praying for you.

Hi,
That's basically what my CRS was like I saw him first in sept last year and he gave me nitro which really did help but only to a certain level, but I tried to ask questions and he hurried me out. Second time more nitro less time for questions 3rd time I insisted he answer me and that's when he said no LIS for women who have had children.
If it wasn't for this group I wouldn't have known how important resting pressure is in getting a fissure to actually heal as he never mentioned it or letting me see what my resting pressure is.
Yes I'm assuming so as my lovely new CRS said fissurotomy while the nurse said oh your here for a scrape lol.
My pain was quite bad the first 5 days but I also have quite a few internal and external hems and I think a lot of the pain came from them. It was very sore when I walked which scared me a bit as walking had always eased it, but by day 10 I was glad I had had it done. I would try it if your fissure is small as I believe I could have healed a small fissure if my bowel behaved. But then again you have already had Botox when your fissure was quite new so who knows...although my new CRS says lots of people heal after a second lot of Botox....I don't think you would be in as much pain as you were getting a sentinel tag removed.
See this is why docs need to listen as we have lots of questions...my CRS advised against removing the hems as it creates more scar tissue and tightening in some people.
I would really recommend going private even if it's just for the consultation because at least then you have your answers

Thanks for your reply. Yeah I think I will go for a second opinion privately. Do you mind me asking which colorectal surgeon you saw? Anywhere near london? I just rang Mr A Windsor who is recomended by a lot of members on this forum but his first available is on 18th aug and my surgery is on NHS is on 20th aug. I really need some more questions answered as well as need to know if I can just have LIS which has a much higher success rate. I can only afford to pay for private consulation but the surgery I d prob wont be able to afford getting done privately.
My fissure ia not as deep or big I think since not so painful but definetly chronic now as its been so long now that its not healing.

I'm in Sheffield, with Mr S Brown who specialises in inflammatory bowel diseases.
But have heard amazing things about Mr Windsor. I'm pretty sure he would do thi LIS for you, I guess you could go on the 18th see what he says and if you decide to just have LIS you are allowed to cancel your operation and see if Mr Windsor can either do the surgery on the NHS or recommend a surgeon who will do the LIS on the NHS for you. I made sure the surgeon I went too also did work on the NHS as I knew there was no way I could afford the Botox to help start the healing as that is way more expensive than LIS.
