Hi there, I'm looking for some advice on what to do about my sentinel piles from people who have been through surgery.

I'm currently suffering from a nasty anal fissure and my sentinel piles (had them for as long as I can remember) have seriously swelled up much to my embarrassment and discomfort. People describe pea shaped piles, but mine are much, much larger than that.
I'm a healthy 23 year old female and am trying to decide whether I should undergo surgery to fix my fissure and remove the piles. I've seen a specialist about my fissure before and he said to visit him again if they become worse. They have become much worse, to the point where I am almost scared to eat anything. I'm booked in for another appointment with him for a couple weeks time.

I've heard some horror stories, but I'd appreciate some experienced advice on whether to remove them or not, considering my age and the severity of the piles. Thank you.

Sorry to hear about your situation. I had sentinal piles too along with AF. It was surgically removed via laser and my anal was dilated for AF relief. I am now at week 10 and almost out of the woods

I suffered fissures pain for 11 months. Was misdiagnosed with Piles. Went through a painful cycle of multiple doctors. My frustration was compounded by every doctor's dismissive nature - (either too casual or too ignorant). No doctor took the case holistically. Some proposed surgery, some said it was not needed.
Finally I went through a strict diet, excercise, stress-free routine for 30 days. I think I am almost out of the woods now. The last colorectal surgeon I saw said categorically that I had NO Fissures now. You would think that would be music to my ears. But after suffering for so long, I am extra cautious. I don't take his word. I am living a life with a lot of caution and care. Its been almost 2 weeks of pain free living now. Hope my 30-day routine helps you. I have written a blog about it here http

It's possible it won't completely heal, but it's highly likely you will get there close to it and will start living normal life. Best wishes to you! I hope NO one, even not my worst enemy suffers from this pain.

Hiya- you are not alone. I have one large (probably around a cm) skin tag and 2 smaller ones around the area along with my chronic anall fissure.

I am definitely going to be asking to have them removed after I've finished my course of Botox injections/ HOPEFULLY rid of the fissure. I've had it since the birth of my son (2 years 3 months).

I'd say if they are getting in the way / causing any pain or discomfort to ask about the different methods of removal. Although I would advise you to wait until your anal fissure is healed as otherwise they may come straight back! :sad:

Thanks for your reply- I actually ended up getting the surgery and it was definitely worth it (for me personally) there was a fair amount of pain during BM's initially, but I would do it again if I had to. Was not as bad as I expected. Everyone's experience is different obviously, but I had an amazing surgeon.
My one recommendation would be to have your first BM sooner rather than later as it's still quite numb for a while afterward. I became almost constipated from the general anaesthetic which lead to a painful first BM with a lot of bleeding. I used barely any pain killers, but I lathered on my nifedipine/lidocaine ointment prescribed by the surgeon.

Yeah I had a sphincterotomy. They basically cut them all off (I was under general anaesthetic) and let them heal without stitches etc. It wasn't as bad as it sounds, but really painful when going to the toilet. I feel like that only lasted a few days though. I had lots of baths so maybe that helped. I only took a few panadol here and there because my surgeons main words of wisdom were "don't. get. constipated.", so I took that very seriously and didn't take pain killers. However I only had mainly external fissures and piles so I don't think I had any incisions internally.

Now I'm about 6 months post surgery and have no piles/tags for the first time since I can remember. I just have some scarring, but from a cosmetic point of view it's 100x better.
I think I have some reoccurring fissures because I occasionally have some minor symptoms, but I drink heaps of water and look after myself better, so I haven't had any pain or new piles.
If I had to have the procedure again (eg post pregnancy) I would.
I can only speak for myself, but it went much smoother than I expected!
