Hi, I've been a lurker around here for a while now.

I've had an anal fissure for at least 16 months now, and I had a botox injection on my nearly healed fissure (the consultant said so but went ahead with botox anyway due to re-occurrence) two weeks ago, however, it is now the worst it has ever been in the whole time I've had it.

I used to be able to cope somewhat with the pain and it would always eventually go after a 30 minutes or so, but now I've spent 6/7 nights in the last week having to take so much pain medication and use ice packs with no relief. It keeps swelling up quite badly in the area, and eventually calming down by morning, and until my next BM. There's horrible bleeding and a tearing sensation every time I go.

I wanted to ask if anyone else had a similar experience? I don't know what's meant to actually happen with the botox injection, I don't know if this is normal and it'll eventually settle down, it's one of those things that gets worse before it gets better, or what. I believe the botox has kicked in as I have mild flatulence incontinence.

This is pretty traumatic for myself, I was hoping this was the end of the ordeal, especially as it was starting to feel a lot better before the botox.

Hi flyingpineapple,

Sorry to hear you are in pain. I had botox about two months ago and my experience was very little pain from the procedure, just a swelling/bruised type feeling. Were you given any notes after the procedure, or were able to speak to the surgeon? Was "debridement" mentioned? Sometimes as part of the procedure the surgeon will clean out any infected or scar tissue, to give the fissure a better chance of healing - basically creating a fresh cut (aka debridement). This could be what you are experiencing. But If things don't improve soon I would contact the surgeon or go to A&E.

The term fissurectomy is also used for what LemonMan describes above.

I had Botox close to nine weeks ago combined with skin tag removal and fissurectomy. The area was very tender for just over a week and it took around two to three weeks for the Botox to kick in but that seems to vary between people.

I had zero pain before the surgery and went back to having fairly bad spasms/pains after the surgery. They improved after around ten days. I'm sure that was due to the fissurectomy and the general area being agitated by the surgery.

It's worth noting that I only had minimal bleeding after the surgery and it stopped after four days. As in a small amount on the toilet paper when I gently dabbed the area before getting into the shower to clean off.

As LemonMan says get in touch with your surgeon or go to A&E as it sounds like you're really struggling.
