Anyone have chronic multiple fissures?

Postby Jimoffy » 09 Aug 2017, 01:44

Hey guys, second post here, exited to hear from all the lovely people I have seen posting and commenting on this site...

Anyway, long story short, I have what I believe to be numerous chronic fissures. I've had pain from the same few spots on my anus on and off for the last 3 years, main one being at what feels like 9 oclock, and another at 3 oclock, and if it's a real bad poop, a couple more little cut pains around my anus ( can't even determine exactly where, the whole anus feels like it rips open with a healthy large BM)

I've been on movicol (miralax) for the last 4 months and decided to have a break because I thought I was having pencil thin stools lol (turns out it was the movicol) and well 2 days after I stopped taking it, did a large nice looking poop for a change instead of the usual soft movicol poops, problem is I feel like I have ripped open every fissure I have ever felt in my anus again... mainly 9 and 3 oclock, and I'm sure there's a few smaller ones around somewhere.

I really thought the laxatives were helping it heal, I saw a specialist last week and he put me on diltiazem parrafin, which gave me blisters and a painful rash around my anus and it's spread to my thigh today, so I am throwing it out.

My question is, has anyone suffered from multiple fissures for such a long time, and it wasn't Crohn's disease or any IBD, and successfully had them fixed, surgically? My crs wants me to do a colonoscopy in the future, but I think I'll turn that down cause I have 0 problems with my stomach or anything else.. just these damn painful fissures... I am a 23 year old male, crying at the moment from this pain.... while on movicol I can be pain free for months, but it's like it never actually heals! The second I do a larger than usual BM, they all come back just at bad as day 1... errr I don't even know what I'm saying right now... too painful... would love to hear a few stories.. I'm really considering surgery.. nothing can compare to the pain of fissures...

Hi sorry to hear your suffering
Have you had a resting pressure test to see if your IS is just too tight. Also could you ask a good CRS to see what the fissures look like. Because although you probably haven't got it, it is possible to get peri anal crohns but that usually includes fistulas too.
I tried the diliatzem first and was allergic to it felt like my whole lower body was being burnt in acid....awful. Could you phone up and get your doc to prescribe some GTN as I found that so much better and it worked very quick.
Maybe you will have to go back on the stool softener
Good luck

Thanks for your reply, I guess there always is a chance I could have Perianal crohns, but I have no family with it, and I first had a fissure very similar to what I have now,(unsure if it was multiple back then though) about 6 years ago. That problem resolved itself after exactly 6 weeks though, the 3 years later I get the same pain back, but this time it just didn't go... as my specialist said if it was due to an underlying cause i would of had more symptoms or complications by now, seeing as though I first had this problem 6 years ago.. but he still said he wanted to do a colonoscopy once (or if) I can get these fissures healed.. I don't like the idea of that, after reading many stories of other people who seem to be in the same boat as me, who had it done and found no underlying cause. I don't have health insurance and don't want to pay whatever that procedure will cost, if it's not going to show me anything but additional pain..

If you have had this for such a long time then it's extremely unlikely to be peri anal crohns as that tends to progress pretty quickly
If you feel that you don't need a colonoscopy then say so as 99% of people find nothing wrong from them. Docs just like to be careful
Could you have Botox or LIS

I think I will turn it down... it's just so painful if I'm off the movicol.. as long as I take one sachet a day I'm pain free.. it's just not something I want to have to take for the rest of my life.i see my crs on Monday because I need to tell him about the diltiazem reaction and see if he can prescribe me a new cream.. may as well tell him I think I have a few fissures instead of only one. Will definetely be up for Botox or even lis... just unsure if these treatments work for multiple fissures? Feels like I have about 5 after yesterday's BM... ugh!

Well I'm seeing my specialist tomorrow, so I'll tell him I can't continue the cream. Hoping for another cream before he tells me I need a colonoscopy and operation lol

Just an update. Saw my collorecstsl surgeon today because my scream gave me a rash, he basically said to either continue using laxatives for another 6 weeks and come see me, thing is I've been taking these for 9 months already and all it does it helps with the pain, as soon as I do a hard one my pain is back. He told me today though he wanted to do a colonoscopy in case I have rectal cancer? Wtf? I asked if it's necessary and he just said absolutely, because he likes to be safe. I don't have any symptoms of anal cancer and I don't have any family with or that has had it.. seems like he wont consider surgery until I have a colonoscopy. Should I see another doctor? I really don't think the colonoscopy is necessary..
