Horible Pain and Anus twitching? HELP

So after suffering for months with horrible fissure pain i thought I seen it all

I had LIS surgery 5 weeks ago,

The first 2 weeks after felt painful but not horible,

Week 3 I had 1 bad poop and now ive been in horrendous pain

I thought i knew what a spasm was but what im feeling now is strangely different, 5 or 6 times every hour my anus will do like a rapid fire twitch then stop,

I'm so confused whats going on.

Additionally the pain seems horrible, Pooping is mostly painless but an hour afterwards the pain pours on and by noon im in cripling pain

Another question is does the surgury permenatly lower your resting anal pressure or just during those first few weeks?

Another questions is i had some minor success having the pain back off from 10/10 to 8/10 if i intentionally gave myself a pure liquid stool almost like diahrea, BUT it would never heal. Is it better to stick to a high fiber diet even if that means more pain

any input is appreciated

Hi sorry to hear your suffering,
I’m not sure what those twitching sensations mean, but unfortunately I know the spasm pain you are describing. I have crohns and it flared 3 weeks after my LIS, this sent my pain levels sky high with constant 24 hour spasms so bad I was seriously contemplating a colostomy.
But they slowed down and my fissure started to heal...yes it took longer than normal but I’m now healed and only get pain at the fissure site after a bad flare.
Could you get an appointment with your surgeon get them to have a look. Also keep up with the sitz baths. If you have any of your creams left you can use them after LIS I certainly did. Another possibility is to see if your dr can prescribe you some painkillers like amytriptiline or gabapentin to get you through the next few weeks or months.
Don’t give up hope you can still heal and you probably already are just a bit slower

What kind of timeline did you have to see improvement?

Can i ask you what kind of diet did you adhere to?

I started to see some improvement for week 8, at week 5 I started to take gabapentin to try and stop the pain spasm cycle I believe this def helped me.
My diet is very different to most people with a fissure as because of my crohns I can’t eat any fruit and have to be careful with veg. So while my fissure was active I basically ate white bread meat and potatoes and a bit of soup. The best thing for me is to get the ideal smooth torpedo shaped Bristol 3 BM as this is non acidic and causes no pain at all not even a twinge

I know im throwing out alot of questions so sorry for that

POST surgery one of the pieces of advice was walking daily but the pain is so severe im bound to the bed, should i be walking through the pain?

When my fissure is really flares up, urination doesnt hurt per say but while im urinating i can feel my anus tighten and hurt, anyone experience this?

Going by the chart bellow right, my movements are more like type 6 or type 7. is this bad for healing?

I was having to work through the pain as I only took 2 weeks off work and I went back to the gym/swimming at week 5/6 although I started very slowly. For me it helped as it gave me a distraction from the pain but I did have to come home from work a few times due to spasms.
I know most advice is to have a BM like cream cheese but for me the watery BMs are way too acidic and they caused my fissure in the first place. Also the softer BMs can leave more residue which will cause the fissure to sting and may lead to spasms. Whilst it’s not necessarily bad for healing it may start the spasm cycle. My CRS specialises in IBD and told me a Bristol 4 is best
