Rectal pain chronic anal fissure

I have posted on this forum once in 2017, here I am again with my second one.

My first post was describing how i got multiple anal fissures and possible muscle damage after constipation.

It took me a year to fully recover without any evasive methods. Over the years had some small issues but got away pretty quick.

After another recent constipation (years later) it happens again, my symptoms are:
- pain level 2 out of 10 during daily BM
- pain after and great muscle spasm level 9/10 for few hours
- a lot of burning sensation inside the rectum with no visible sign of a new fissure(old sentinel pile present)
- while trying to dilate i can feel probably the scar tissue going 1-1.5cm up inside the rectum and most of the times there is pressure coming from the the ‘walls’ groin side but only sometimes i feel a ‘clear path ‘ ahead. - might be the prostate idk
- passing gas hurts and triggers the pain aswell (feel the cramps on whole colon when the gas passes)
- for some months now i got stress and i feel great amount of pressure in my pelvic floor, impacted my lifestyle and my core posture leaning ass backwards a bit.
- impact me on the bladder side where i feel like peeing often after/small incontinence
I fight this for 1.5 weeks, i do sitz baths that do help with tense muscle, use creams 2-3 times a day, self finger dilation , relax during bw for best elimination.
My concern is, as said, during dilation i can feel pressure or pushback beside the fact That everything might be internal and no fissure signs on the anus…
Abdominal ultrasound ok but they dont see bowel, need further tests. A bit folic acid deficiency, a bit high hemoglobins red cells .

I found relaxing the perineum muscle way effective with the spasm. Attached guide
