"Pressure" or straining is due primarily to various degrees of constipation, and most people get their fissures from hard stools cutting the anal membrane, the stool having dried out while constipated. The other main cause is severe diarrhea, which can cause trauma to the anal membranes also.
Other mechanical causes like rough medical examinations, anal sex, etc. can cause fissures, which occur near the rectal opening.
