How do you know when painful is bad?

Hi guys (and belated Merry Christmas!)
I've not been writing on here for a while as work has been super busy and have been soldiering on with movicol every day and anoheal. I'm not healing, I'm still in pain every time I have a BM but I can work and walk and sit and socialise so I definitely don't have it as bad as many on here.
This week though I've started to have a lot of pain and I'm not sure if it's from a retear or something worse. Normally if I have a retear I'm not in pain the whole day, just during and after I go to the loo. This feels like this sort of 'full' feeling, its kind of hot and burning and sore all the time. My skin tag has swollen up to double its normal size and I also have this small kind of painful lump near it but more on the outside. I don't think there's any discharge but I'm a bit worried it's got infected. It does feel worse than a retear as it's more continuous and I'm getting paranoid about abscesses and fistula's...
I have an appointment with my doc tomorrow morning but if someone with any similar experiences could let me know what they think, it would be amazing. I am so used to day to day pain with this that it worries me that it's suddenly got so much worse.

Hi Layla..I don't know how long you have had your AF, but it sounds as though it may be getting chronic. Sounds like you are having the spasms that go along with the fissure pain. Glad you have an appt soon!!

Thanks for your reply Sue :)
Sadly my fissure is definitely chronic, I've had it for a year or so now :( I've been using anoheal but my CRS is very reluctant to do anything else for a while as I'm 26 and haven't had kids yet (he said if I was a guy he'd have no problems offering it to me now).
Tbh although I initially thought he was great, I'm thinking about getting a second opinion from somewhere as he's said if anoheal doesn't work he'd rather do the stretch operation (anal dilation) than LIS. I've heard that the stretch can cause incontinence and is generally considered to be really out of date so am incredibly against going for that over LIS. In addition to this, when I asked him if it was ok that I'd been relying on movicol for over a year, he told me movicol wasn't a medicine so that was fine! I'm pretty sure it must be as you have to get on prescription or over the counter.
Really hope my GP is more helpful tomorrow...

Hope you have managed to get some help Layla. I am now 6 weeks post botox and only just seeing a difference but thankful for that small mercy. Just wanted to say aswell thank you for your advice about the clinic regarding my other problem! (the thrush - or not thrush situation!). I went to the clinic over christmas and they are confused as to why I was treating myself for thrush as they said every swab done on me has come back clear. They've done another swab to be 100%, but not expecting it to show anything, and have referred me to a dermatologist so hopefully can get that sorted. They think its contact dermatitis. So thank you again! I really hope you can get something sorted out, sounds like you are going through it again at the minute. Thinking of you :)

Hey Sue, good to hear from you!
I'm not great to be honest. I've been lucky up until now that despite the fissure not healing, if I take my meds and am careful its generally not consistently painful. i can't even sit down at the moment without being in agony now. I'd like to take time off work but I've moved out and have to pay rent and my job is getting more and more demanding...It's rubbish. My GP says the fissure has got much deeper which is why it's more painful. She's suggested a fissurectomy as well as what the CRS has suggested (anal dilation eeeek).
I have an appointment with my CRS on the 19th and think I will just have to hang on til then because he's away and the hospital say I can't see anyone with else without a proper referral which could take months. Nightmare! But I'm trying not to dwell on it too much, and my parents and friends and boyfriend have been amazing and so I'm grateful I have so much support in this.
Really pleased you went to the clinic! :) They see stuff like that all the time I think, as so many people go in thinking they have the symptoms of an STI when it's actually something else. I think you'll get it sorted really soon, I was so shocked mine cleared so quick once they'd worked out what it was. Canesten is evil as well, I'm never going near it again!
I'm curious to hear about your botox. Do you think you'd recommend it all in all or is it too early to say?

I'm sorry to hear you are not good again Layla. I thought they said they were going to do LIS if the anoheal didn't work? They're not keen on this op are they?! I was told I wouldn't be offered the LIS until much further down the line. Hmm, anal dilation, got to be honest I don't know much about that! I know what you mean, my family and friends have been great too! Hopefully on the 19th you can get some help or at least answers. A fissurectomy may be good. Hmm has the botox worked. Well its difficult to be sure as I retore straight after it which didn't help. But this last week it has improved loads! I have almost no pain during bm, twinges of pain in the day sometimes. Only get afterburn if I'm unlucky or have a particularly bad day. Overall I had horrendous pain for 2-3 weeks after if I'm honest but things have drastically improved. Not right but much better! Mind you I have just had 2 and a half weeks hols, so going back to work tomorrow will be the real test. Was ok ish on Friday but no kids in as it was a teacher day! Can you not ring up sick for work for a few days? At least give you a break. I don't know where you are to suggest anything, but I'm in Birmingham so could give you the names of people round here if thats any help. Take it easy (know that sounds silly but I mean look after yourself!), and I've got my fingers crossed for the 19th for you. Let me know how you get on :)

Sorry just realised you asked me if I would recommend the botox, erm I think its a bit early to say. All I can say at the moment is it does give you relief eventually! Will update in a few weeks! :)
