Hi all
I have come across this forum after suffering from fissures since 2008. Can I just say how what a brilliant site this is! :)
I have suffered for years with my enemy Mr Fissure, I have been on creams, suppositries, GTN the works. My fissure hasn't healed properly for a good year now, other than when i had my first lot of botox in feb, when it eased off for 3 weeks. I always feel it rip and normally try not to have a blart fest in the bathroom. I have piles internally, skin tags (what are these lol?!) and this lump externally that likes to swell up and down all the time (maybe a shy pile lol not quite sure?!) I have been using a pile cushion since march but it doesnt do too much :(
I am booked in for a second round of botox, removal of piles and skin tag. I am just waiting for my appointment to come through. Has anyone else had botox twice? My consultant said because the fissure is old I would have to have it scrapped first and then botoxed so its a fresh wound to heal. Has anyone else had this done? Any info would be appreciated folks :)
Im doing my best to be cheerful about the situation, I can be tearful at times, but thanks to this website I have hope now!!

Hi!
My colorectal surgeon (CRS) actually recommends against Botox as he says most of the time it just delays the inevitable (i.e., surgery) and prolongs the discomfort. In fact, he will only give Botox if a patient demands it.
I am now a couple of days post-op (I had a fistula repair and LIS) and I can't believe I waited so long to have the surgery.
While I am not a doctor, and I don't know your specific circumstances, I would strongly recommend you consider LIS and skip the second round of Botox and be done with it once and for all.
Also, if you aren't seeing a CRS, I would strongly recommend you do so as you want a specialist that deals with these issues day in and day out.

Botox advice

Hi!
Thanks for posting :)
I am currently seeing a consultant in the rectal clinic at my local hospital. They dont seem to keen on doing surgery here. They seem to think botox is the solution for everything (either that or its the cheaper option). This concerns me to be honest lol! I am not sure if they will do surgery but will ask when my appointment comes through about all my options.
I am glad your surgery has went well. Its soo good to hear someone is feeling well and is getting somewhere positive :) I feel like no one in my family/friends understands, so I tend to suffer in silence lol. Let me know how you feel in a couple of weeks :)
Its been really hot today and I am not sure if this has contributed to me feeling like hell, fissure and piles are testing me tonight! Tomorrow will be a long day!

Hi again!
I think most of us on this forum know what it is like to suffer in silence. Unless someone has gone through what we are going through (or have gone through), they can't possibly understand the pain or despair we feel. That's the great thing about this forum - you will find lots and lots and lots of wonderful people here who know exactly how you feel. In addition, there are some amazing people on here from the UK (Davo, Ever The Optimistic and Sue27uk, for example), so they may be able to offer you more specific advice than I can since I am in the U.S.
In regards to surgery, do you have the option of going private?

Hi UK fissure chick. I know what you mean about the botox thing, here in the UK they kind of seem to favour it over the LIS as there seems to be more concern about incontinence. I don't know how old you are but they tend not to like to do the operation on women who are of childbearing age. I had botox in November and found it really painful. Went for my follow up in May and she said she couldn't see anything and wanted to send me for endoscopy. To be honest I hadn't seen this lady before (as I seemed to get different people each time!) and her examination was the most painful and I bled for 3 days afterwards. After almost 18 months of this rubbish I decided to go private and its the best thing I did. He found my fissure had healed on top but not underneath so had formed a fistula. I went ahead with the laying open of the fissure/fistula and more botox to improve healing. I would recommend getting a second opinion, or maybe going for a private consultation. You can then be referred back to the NHS for treatment. Make sure you get someone who you can do this with if you go down this route. Other than that I would suggest asking before if they can 'clean up' the fissure site when they inject botox so there is a better chance of healing. This is what my CRS did, he laid it open, made sure the area was clean and then injected botox. I'm not sure how useful this babble has been!! But if you have any questions let me know Image

Thanks so much for posting :)
I totally understand what you mean about them not wanting to do surgery. I am 28. My concern is that there is a fistula there. I had 2 major incidents when i was on the loo. It always rips but these two times the pain made me scream and i was shaking. I mean i literaly was in a right state. Because I have had it soo long, i am concerned that it may be a fistula and have tracks be deeply imbeded in there lol. The consultant said they would scrape the fissure out first to make it a fresh wound and then botox it, so it would have the opportunity to heal and knit well. The botox helped last time for 3 weeks.
I feel that you really have to be harsh on the consultants here, they dont seem to want to listen. I will see what they say at my next appointment. I cannot wait to get it sorted, every single day i am in pain. I work in a office so i am sitting down all day, this makes it worse. If I walk for 15-20 mins I swell up and am in agony, so i have had to pack the gym in for now :(
How are you now after surgery?
I am determined to get it sorted!

No problem, I have been there and am still going through it! I am also 28. I found some of the consultants/registrars/doctors not very supportive but others have been. Seems to be a bit hit and miss. If I'm honest my surgery was 3 weeks ago and I am still in quite alot of pain after bms but this seems to be improving gradually. But at least I had it done as it wouldn't ever have got better on it's own. I am glad I had it done now, hopefully things are slowly improving! I am going to my GP about an ear infection tomorrow so will just ask about it still being painful. But going back to work really did me in, went back too soon now I see that. I work in a school as a Teaching Assistant so kind of have to hide it from the kids when i'm in pain. Lucky that I'm on hols now so time to recover!! It's good you're so determined, I really struggled when this first happened, but now I am determined also to get better. Let me know how you get on and keep me updated Image
