How to apply Diltiazem cream?

Hi guys
After a very painful 48 hours and an emergency hospital stay I find myself 'blessed' (ahem...) with an anal fissure.
I've been prescribed Diltiazem cream (2%) to apply twice daily but wasn't given much direction. I've applied this twice now but both times it's been burning to the point of pain. I've been applying it on the outside ring of my anal opening... am I applying it right or do I need to go deeper?
Any advice? Right now I find it easier to move about etc without the cream and I doubt this is what the doc intended!!

Apply it on the anal ring and most important one inch inside.

In all honesty is the relief that the cream brings worth inserting a finger up there? Normally not an overly painful act but in my case that's gonna be bordering on deathly!

Thanks guys, started to apply it using a Q-tip today and for the first time in 5 days it's been bearable enough to have a BM! Lets hope this is the start of it getting better...

I heard from multiple CRSs that you should not apply inside. It actually makes no sense medically speaking - the skin absorbs the medicine and it goes right to the fissure if you just rub it outside around your anus.
The problem with applying inside is that you could hurt yourself (like I did when I was applying inside) and you will be getting no benefit from it. Diltiazem is not like Neosporin, putting it on the fissure itself isn't helpful.

I have read that people keep their diltaizem cream in the refrigerator. I was never told to do so and it is not on the prescription label. I have had the cream not in a fridge for almost a month, any chance it has gone bad or something? It does not seem any different.

Unlikely it has gone bad - it does not need to be refrigerated. It may become less potent after 6 months in the tube/jar (that is what I was told) -- but most likely you will have used it up before then.

Thank you. The cream has an expiry date on it. Still over a month until then. I think the problem I have now is a painful lump just outside the anus. It stings like fire when the cream eventually touches it.
