Gosh, there are so many different things to try. It's really great you are getting some extensive evaluations done, which will help shed light on your muscular function. Pelvic floor physical therapy s one alternative approach that could potentially help.
Chinese acupuncture and herbal medicine is another.
Self dilation at home using graduated anal dilators is another that a few of us are trying out. My CRS has also suggested my trying this.
My GI doctor suggested trying daily enemas for a few months to see if that would help. I haven't gone that route (yet).
I have tried various topicals with varying degrees of success and failure.
My diet is impeccable with no cheating in terms of what works for my body.
I take supplemental zinc, vitamin C, magnesium, and eat lots of colorful veggies and fish for their wound healing properties.
I use a modified squat position on the toilet to reduce the pressure on the anal canal, and never ever strain.
You may wnt to consider switching to Miralax, called Movicol in the UK, if the sugar is concerning to you. Other than the blood sugar issue, if you have any yeast contributing to your fissure situation it might be a good idea to think about switching.
Other alternative therapies I've heard of are light therapy, hyperbaric oxygen therapy (though this would be super expensive), seeing a naturopath for specialized herbal formulations, etc.
I pretty much have to be open to everything because the doctors I've seen have all advised against surgery for me given my other health problems.
