New to the forum: chronic AF on/off for 20 yrs...need help!

Hi,
I'm 37 and I've been suffering on and off since I was a teen. Up until I had kids and moved overseas in 2007, the pain was on and off and tolerable. I saw a nurse prac at a GI office today. He took a look and couldn't get very far due to the pain, but said the fissure is huge and one of the worst he's seen. He recommended surgery and hemm surgery as well. Problem is, I had LIS done while we were living overseas in Spain. It didn't work and I've been much worse since...that was in 2010. I had a bad infection following surgery and immense pain for 3mths following. It was horrible. The incision site never healed completely, the fissure grew in size and depth, and I now have a tighter mucscle. I am now trying nipedine oitment but so far no luck (been about 2 weeks). I now have a new critter down there - an external hemm lump. If anyone can advise me on a second surgery please help!! I have 3 lovely kids to take care of and right now I can barely sit. My life revolves around poops and baths. It's very depressing!! Please help if you can.

Believe me, we can feel your pain Image So many of us have been or are in the same situation. It is very difficult to say what to do, but I would definitely suggest seeing a CRS rather than a NP for this. And the best CRS you can find.

Thank you! I have an appt next week - 3 actually. One with the GI, another with a CRS, and also a nauturopathic doc - worth a shot. I will update after that.
Anyone had luck with cauterization with silver nitrate?
Has everyone been told you have to have a colonospocpy before they will operate on you? I can't stand to think of how painful the clean out would be!

Hi ROF,
Aww so sorry you have been suffering for so long. And I understand how difficult it is to even function with an af, let alone run around after 3 kids..(I have 3 myself).
What I would do is take a list of questions along with you to your apptments and go through them all so you fully understand your options..the expected pain associated.. And recovery times.

So sorry to hear how badly you are suffering, and epecially with children to care for.
There have been some people who have had success with silver nitrate, though the reports are that it's pretty painful, and with a fissure as large as yours I don't know how effective it would be on its own.
A colonoscopy is really not necessary before surgery though there are doctors who prefer to do them. It really depends on the doctor.
Will be really curious to hear what the naturopath has to say!

Thank you all for the words of encouragement and advice. This condition is hell for all of us. I will update this week on the appointments.
I plan to see a 2nd surgeon as well. The office I am going to this week sent me away back in 2010 when we moved back. As soon as I said "I just has LIS overseas and it didn't go well" she basically shooed me out the door!
FYI - I stopped trying to put my gloved pinky inside "the ring of fire" and just outside, slightly in. It feels better today without the added trauma of my pinky going in.
Hope this helps someone else - Continuing: Miralax 20g, many hot baths, nipidine/licodaine oitment but on the outside/just barely inside 3x day, coconut oil a few times a day used on the outside, cod liver oil ingested, and an herbal tea with slippery elm bark. Also stopped eating red meat and coffee completely, and dairy almost completely.
