How painful are AF?

Hi
I know that this is a strange question. I had surgery almost 3 months ago to repair a fissure (or so I thought), abscess, fistula and hemorrhoids. During the last month, the pressure and pain increased to the point that it hurts to walk, cough, move, sit, sleep on back etc. I saw the original surgeon and they said that I have another acute/chronic fissure. I suspect that he didn't repair the first one although he said it is another one. I don't remember the first fissure being this painful. Maybe because I was fighting with the abscess at the same time.
Can fissures become so painful that you have to take pain meds to get some relief? After I go the bathroom, I get pressure and pain so bad that it feels like everything down there is in spasms. The pain even moves toward the front area (vagina)
The surgeon wants to do a LIS. I am very hesistant in having the same dr do surgery again because I am frusturated that he didn't take care of this the first time around. He referred me to another CRS who apparently has more experience for a second opinion. Any ideas on what I should do?
I am so tired of being in pain down there.

No. He apparently cut out or used laser to remove the fissure.

Yes fissures can be incredibly painful. When mine first occurred, there were times when I thought the pain would drive me out of my mind. And it follows that yes, they can be so painful that you have to take painkillers. I did on a regular basis. I took a prescription strength NSAID and paracetemol together, every day. Sometimes just in the morning and sometimes repeated throughout the day when the last lot wore off.
I can understand your hesitancy around the first surgeon. If you don't feel confident with him then why not seek a second opinion. It's what I would do in the circumstances. I saw three CRS's in all and only the last one fixed me, and that was with the LIS. I'd add that my only problem was a fissure and the muscle spasms and increase in muscle tone in the internal sphincter that came with that.

Hi Jen,
You had a lot of work done down there.. And that may have led to another fissure being formed.
I first got my fissure after surgery for hem banding and tag removal.
If you are not happy with your current crs, I urge you to find one you feel comfortable with and trust.
It must feel like such a let down for you after all you had done. Xxx

Yes I've been healed a long time because my LIS operation was 23 months ago.
I didn't have the typical situation of an open fissure which healed following LIS. In June 2009 I developed a 1 cm fissure which was down to the muscle but it healed very, very slowly over 5/6 months using Diltiazem. After 5 months I became allergic to Diltiazem and had to stop using it. Every time I went to the CRS during this 5/6 months he would report that the fissure was healing and he thought it would heal on its own, yet I was still in pain. Even at the point, 5 months after it occurred, when he told me that the fissure was now no more than a 'dip' and could not rightfully be called a fissure, I was still in pain. At this point he referred me to another CRS for Botox because he thought it was the muscle spasms causing pain. The fissure was just about healed. When I saw the second CRS, the fissure was completely healed but I was still in pain all the time. The second CRS gave me two lots of Botox at six monthly intervals, but I was still in pain, both during the time when the botox was there and definitely after it wore off, at which point she seemed to give up with me. She never once mentioned the LIS. So I went to a third CRS who thankfully saw what was going on. Whilst the major fissure had healed, my anus had become very tight and the muscle tone high with all the spasms and it had become very reactive. Because it was so tight, I was forming minor fissures with most bowel movements and because it was very reactive, it was spasming in response to these minor fissures - hence the pain. So in April 2011 he performed the LIS. It took a while for the pain to gradually diminish - I was not one of those who were virtually pain-free immediately after the procedure - but it did diminish and my anus returned to normal. I'm leading a normal life now. No more trouble. :)

My solution that worked....Nitro cream 4% and milk of mag..sponging off over the toilet with warm to hot water and Aleve or Naproxen.....that combo helped me a great deal.....of course fiber and water throughout your diet.

Do fissures hurt so much you need pain relievers?
Honey, fissures hurt so much that even with pain relievers, you might someday find your self doubled over, crying for your mommy because the pain is so bad. Fissure pain made me throw up and almost black out. Fissure pain leads otherwise intelligent people to put honey up their butts in the hopes that it will end the pain. Fissure pain makes us decide that its a good idea to let doctor's cut our sphincter muscles in half to relieve the pain. Fissure pain makes us grind up heart attack medicine and apply it, with our fingers, to the INSIDE of our a$$.
In short, yes. Fissures hurt.
