Hi to all, im following this forum for a while and first of all thank you guys for make this forum and support all people with this problem,sorry for my english im not native english, my story is: im 27 years old from czech republic and i was always a lover for chocolate, around september i go to the toilet and push so damn hard, that my ass crack up, first two weeks was a real pain , i sympatize with all of you in this, is it a reaaaaly terrible pain, i m for two years vegeterian and when this problem show up, i start avoid also glutein, i eat very rarely glutein, unfortunatly in my country theres no nitro or calcium blocker creams, dont know why, so im using oak bark cream thats what doctors here prescribe and lactulose, but what really help me a lot is apple fiber, i manage the last 8-9 months get absolutly pain free 3 times, i got always for 2-3 months absolutly no problem but after that time 2-3 months i got the BM little much harder and the problem comes again, so right now i dont know what to do , i mean i was 3 times for 2-3 months problem free but always the problem return, right now luckily the last 5 months even if the problem return i have no bleeding , but when i got BM its really unpleasent and stings, i want ask your opinions should i go to a surgery if the problem keeps returning?or keep on diet and natural cream and hope ?BTW in my country they make a chirurgical surgery called ,,violent expansion sphincter,, or i heard some people got a kryoterapy, like when they put you away a wart, but its only on private clinics and costs a looot of money, to be honest im kinna desperate, last time i got three months whitout problems or pain and now 4 days ago i got a little harder stool (i really dont know why i keep having the diet, eat a lot of veg and fruits, drink alot )and the fisure crack up , i know in confront from some of you is not that bad , but i really dont understand whats happening why is getting still back ??can you give then some of your opinions and maybe hints, Image

We're sorry about your bouts with fissure but you will find some answers here.
Had mine off and on too for over 10 years. But I beat mine with diet alone. You can too in many cases if your determined enough.
I also could not understand what I was doing or eating that caused fissures. But through long trial and error I found out what combo of foods to stay away from.
Stress, lack of exercise, lack of water, too much red meats, too much starchy junk food. Its a guessing game trying to figure what we did wrong, but keep trying.
In the mean time I use the creams and lubricated with petroleum jelly before each bile movement. then showered to cut the burning. Also pat dry don't wipe after bm.
If my bile movements were really tough i would resort to a liquid diet to give fissures time to heal.
Try to find in your diet what makes your stools hard and soft. Use a stool softener like miralax if available in your country.
Increase your water intake and exersice and this should help.
you will get much more advice soon so stick around for a while. I never went to a doctor and am now fissure free for well over a year.
Please weigh your decision wisely before going into surgery.

Hello MichaelJ, and welcome to the Forum!
You said in your post about "violent expansion sphincter," which sounds to me like the Lord's anal dilation, which is not the most used method for anal dilation these days. What is used (including at the Cleveland Clinic by some colon and rectal surgeons (CRS)) is Standardized Anal Dilation.

Hi to all,
today i order myself to an lord stretch procedure, my father had it 5 years ago and told me it helped him to get rid of the anal fisure, in my country they dont do the standardized anal dilatation, so i really hope im gonna finally get rid of this horrible problem, at thursday im goin to the procedure im gonna let you guys know how it goes , if i dont, it means that i jumped under the train :D this the worst problem of all in the world i think

Mate, Im sorry, but I feel I really need to tell you this.
The reason many countrys dont offer lords dilation any more, is because of the higher risk of incontinence compared to modern methods. (controled standardized dilation/LIS)
Please research this before you have it done.

damn, man you shouldnt tell me that now i really dont know what to do, last week i was in so much pain i almost kill myself , after 8 months i cant live like that , i admire people who say that they fight with it 2+ years , i really dont know what to do now :(too bad i cant buy a gun on the street like in usa , i just want end my life this is the worst think that happened to me

okay guys so i get really pissed of this situation and call wherever i can, this doctor where i should go i told him i didnt want to do the lords stretch and he told me to call a proctologist specialist and i we had also a big argue, so i get rid of him and call my friend to a hospital he told me that tommorow he will try to make me an appointment there so i reaaaaly hope they gonna make me the standardized dilation, also i had a question for you guys?i was on a cololonoscopy 4 months ago they told me my intesint is allright and i had a fissure and very small hemeroids, is possible that like i said i got two times no problems for 2 motnhs and then returned , that i dont have a fissure anymore but a hemeroid instead? i so freaking out because i got a chance to go out my country to italy and start a new life there, but i was there a wee ago for 6 days and thanks to the glutin(pizza, pasta, white bread )+fish+ ice creams i got the problems back but after two months of piece is posiible then , that the fissure is gone and i got hemmeroids instead?or is just the fissure reopening?

Like I said, I am truly sorry!
Im not saying you shouldnt do it, I just want you to research, and be aware of the risks. Maybe your doctor is very good at it, and there is nothing to fear, but you need to talk to him about it.
If you have burning spasms for hours after a bowel movement, its most likely a fissure. Hems are usually not painfull unless they get thrombossed, and even then its more of a deep throbbing pain.

I hope you get the appointment to see a new doctor, and good to see that he is a specialist. I imagine that they do Lord's procedure with a different method than in the early 1900s...
A fissure can be far more painful for its size (often the size of a paper cut, like on our finger, or the cut we can sometimes have on the side of the mouth). There are so many nerves in the that area, and it is the same for all of us that have this thing.
I did not have a colonoscopy, but only a sigmoidoscopy, where they told me I had large hemorrhoids...but, it is difficult to tell you if the hemmies are giving you the trouble, or if it is the ifissure. I go with the fissure, which can re-tear, and give more pain for its size than having a baby!
Hemmies usually do not hurt constantly, unless you have a thrombosed one, which can hurt like hell. As JHH said.
I suspect that eating pizza, etc., has brought back your fissure, but only a doctor can tell you if that is the case.

By the way, have you tried soaking your rear end in a Sitz bath, or just soaking in a bath, to relieve the pain.

thank you all for the support , about the pain i got it always one day only, even if i had after the reopen a hard stool i got the pain only when i first push the stool and little pain after goin out toilet, then i got no pain, no itch , like i said i had two times after two months little harder bm and then i got one two day pain then start getin softener and after a week i m again ready , this scenario was the last two times, but also i must mention that the pain is on the side where the fissures was, so im guessin i got still the fissure, and one more question i know some people say that they had 6 or 10 years the fissure, when you got the cut and bleed , can you get some infection in it??the stool with a lots bacteria pass the fissure isn it dangerous???
