Very, very good shout ETO, I know what you mean - I am going to try that! Only problem is you have to guess how much is going up....ah well this whole thing is a guessing game anyway right.
Peace and lure

my CRS said outside is ok. It hurts so much worse to put it inside. I am not sure what to do

If the CRS says do it, it must be okay?! There is so much mixed feedback about this.
I have been using the sort of tube thing that comes with Anusol to apply mine the last few days. It is not ideal because I can't see exactly how much is going in but at least it is a bit easier for me.

PLEASE READ:
You have to put this cream inside your butt. It will not work on the outside. I have been prescribed this and I was told to use it "Internally".
The reason Diltiazem works is that it stops the sphincter muscles from spasm. The muscle spasm is what delays healing and causes pain and sometimes further injury.
If you use this on the outside, you will be doing NOTHING to stop the sphincter muscles from contracting and pulsating.
The trick is doing this without hurting your fissure. The best way I can tell you is:
Use your pinkie finger, put a generous amount on the tip. Stick it up your butt juuuust enough that it is going inside. Then, simply sit down. I think the act of sitting down spreads out the amount of cream and it will cover all areas (including your affected ones).
This has REALLY helped me. I am getting better all the time (as long as I eat tons of fiber and drink a lot of water). Using this externally would not have helped.

Hi,
Have to TOTALLY agree with the above post....
Exactly how I was also advised to use the cream and for the same reasons given...
It also did a fantastic job on helping with the fissure.
WW - glad that the Anusol syringe is working a bit!! Hope it is helping in the healing process for you x x

On another thread someone mentioned using a 10 ml children's dosing syringe. That's been working for me perfectly! Can avoid getting much on the fissure itself much, the gel stings a bit at first.

I was told by my CRS to apply the diltiazem (mine is in a vaseline base) outside and a little bit away from the anal opening.  I was also told that the diltiazem was never meant to work as an analgesic (soothing pain), but instead is a drug that stimulates blood vessel growth, which in turn gives the fissure a better blood supply so that it can heal itself.  By applying the ointment externally, and a little bit away from the anal opening, it can be absorbed through the skin there and then help blood vessels become larger in the whole surrounding area.
