I just read your earlier post. If it makes you feel better my fissure is 1.5 cm. So 2-3 mm counts as small I guess.
So what happened with options 1-5? Did you do any of those?
I did something similar to your option 5 (kryotherapy) and I regret it ever since. I did cauterization and it just aggravated my fissure, multiplying my pain many times. It took me weeks to recover. Might work on small fissures, but i would never do it again, although my doctor still claims I wouldn't have any chance of healing without it, due to the scar tissue build on the edges of the fissure, that needed to be removed.
Luckily you have 4 other options. Botox worked for me.

Sorry you're dealing with this terrible problem! And glad you found this website for support and ideas! When I first found out I had an AF and what the treatment options were, I wished I were dead at times, too! None of them (except the creams) sounded tolerable. As I'm getting used to the fact that I do have this problem and need to find solutions, I am glad there are specialists and procedures that can help! I went to a naturopath who has treated hemmies and AF's for 37 years. He uses a dilation approach with his fingers (is that the Lord's method?), cuts away scar tissue, and burns away tags (I believe, called diathermy). I haven't had that done, but a friend had it done with him and found it successful and not too bad to recover from. This guy told me my AF was huge, about 2 cm long and 1 to 2 cm wide. I wonder if that is accurate. Anyone else have AF's like this? He said mine was among the more severe he sees, though not necessarily his top 10! It was a little scary to hear this! Anyhow, I also have seen a PA (physician's assistant) in the CRS clinic through my health plan. He could only tell me I had a fissure and offered the creams, and LIS surgery if I wanted to pursue that. He didn't mention botox until I asked and then I found out some of the CRS surgeons will try that. So I am scheduled to meet the surgeon later this month. In the meantime I have researched extensively, on-line and read many of the posts here. I have improved pain levels by figuring out how to keep my stools softer! In the beginning I was told 'high-fiber diet' and water (and psyllium husk and colace type softeners). I still had some firmer, hard-to-pass stools until I lessened insoluble fiber and focused more on soluble fiber. I learned that here. I've had three children without major pain relievers on board, so I can vouch for how painful (and similar to childbirth) an AF can be! Whatever treatment you decide on, it is important to figure out how to maintain better consistency stools for a long, long time to come. The PA said he sees some people who have retears even after LIS. So he emphasized life-long stool management. Read some success stories on this site and give yourself time to figure this out. Man i m waiting for the next week to start the nitro and dilatezem cream, but after been absolutly pain free 2 months and then reopend and this happend to me like three times, i see, like i said, that if i dont drink lasatives or eat vegetables and fruits all day and got a little harder stool i never get rid of this problem, right now after the 8 months of the fissure i got 68 kilograms and 192 cm , im on the edge of malnutrition thanks to this diet, so i need to eat much more and get up some kilos and that meens hard stool, i stop using lasatives because after 4 months on miralax i got incredible stomach pain, i cant drink it more, so im gonna have pain again and again, at least if i got stoned after weed i dont think of the problem too much and get up some kilos, when i read stories of some people here i didnt find that they go back to eating pizza, pasta, red meat or other stuff, everybody must had incredible extreme diet so the stool not come harder and the fissure not crack , logically if you got on a muscle a scar and push that muscle even after a year the scar can crack , practicly there so no way to get rid of it forever , you gona have always a scar there even after lis gonna be a scar there and if i must chceck myself everyday like a model at least i want enjoy a little this life , yesterday i get high i laghued my ass off for like 4 hours , really i didnt laugh so much for years then i fall asleep and today morning even the BM go suprisingly better, i dont want affect anybody with this comment but weed make my problem better and im gonna try next week this http://en.wikipedia.org/wiki/Dragon%27s_blood dont know how scars in the ass, but my friend told me he got a nasty 2 cm scar on the face and after applicating this went off totally after 5 days , im currently lookin and calling specialist all over my country so they can make me a standartized anal dilatition but i cant find anybody , and goin in some other country will cost me thousands of dollars which i dont have

Hi guys , i must apologize for the recent outburst, im just so desperate of this situation im 27 years old and im scared about living the rest of my life with this all restrictions, now two days after smokin weed and using dragons blood i m absolutly pain free, last two days i had absolutly pain free BM with no spasm, but i now last time i got 2 months without problem then i got little harder stool and the problem came again, thx to buttastic i got still some hope that i can get rid of this, this thursday im goin to the CRS to start the cream pill treatment i also heard is one of the best CRS in m y country, that he travel to US and all over the world i got printed the website from wecc about SAD and i hope is gonna tell me that it can be done, once again sorry guys for the outburst and thank you all for helpin me treat this problem , thank you

Hi guys , its been a while i got here , im again back to share my progress with other fissure soldiers :), 7 weeks ago i go to the specialist proctologist in my city, he did it an anoscopy and tell me i got 5mm fissure , i start the dilitiazem cream, in my country is still i phase os clinic studing so i go to visit him after two weeks i got no pain at all but he tell me i got still 3 mm fissure so i continue the cream oinment , i go visit him again after two weeks he tell me i got just little small 1mm fissure now its been three weeks and i go to see him next weekend for the last visit, im pain free all the time, but last two days and last two BM i got a little harder stool i dont have any trouble but i sense today a n irritation during the first push of BM, im doing the prescribed diet all the time and i dont understand whats happening , i really hope the fissure didnt open again , i read almost all the topics is the diet section ,guys i really hope the fissure is healed and next weekend hes gonna say that is fully healed its gonna be one year of trouble in two months i really want to get rid of it , do you think its healed and now i got only this irritation during BM cause of the scar ?like i said when i go to the third visit he told me i got still 1 mm but i dont had any pain at all, im really confused seems that 1-2 mm fissure doenst hurt but is still there, im really very confused guys:(
Update:today i got again a little ahrder BM and i got a litlle bit of blood on toilet paper i dont have fortunely any pain but the fissure is definitevly back, i was sooooo close to cure it, guys im on the edge of suicide , i dont know what to do anymore

Please no, I know easier said then done..I have felt like I wanted to die to..when I'm in intense pain, I cry and repeat Please go away (hense my name) and then." I say, I don't want to be here any more."..then the pain slowly subsides and I get on with my day or night. Please keep your head up..we are here for each other ..Please.

Please don't kill yourself for a 1 mm fissure or any length! You see, you are actually achieving great progress, you decreased your fissure with 50% every two weeks, going from 5mm to 3 and 1.
What I experienced recently is that pain is proportional with stool consistency. My fissure went from 15m to 5mm, but when I got a harder stool like Bristol 4, it hurt just as bad as when the fissure was big. So even if your fissure is now smaller, the new skin around it is still very thin and irritable, so it can cause pain.
So don't despair, take your diet to the next level, so you never get constipated again. It is doable. You are so close to a full healing, 1mm is like nothing really. Make sure you always get Bristol 5-6, to me that is the best consistency. Although Bristol 4 is considered the best stool consistency on a healthy anus, it doesn't work on fissures, in my experience.

I also agree with bcg, A 5-6 works for me too. not a 4 even though they recommend it, could be the scar tissue, either way its more comfortable and no or very little spasms/pain and the like.
By increasing my soluble, (about 60 to 40 ratio) it makes my stools on the softer scale, but its always trial and error every day. so Please don"t give up.

Dont let a little fissure beat you mate. We all know its a rollercoaster, so please remember to look at the whole picture, instead of just the low's. Yes, we will have small retears, but we will eventually cross the finish line, so hang on!
- Fissure developed in Jan '13
- Started rectogesic in Feb '13 and diltiazem Apr '13.
- Got botox Jun '13
- Healed by Okt '13, although I still had some irritation for about a year.
- New fissure April '22, healed June 2022
- New fissure 24. December 2023
