Try heat/warmth to the buttocks. Not a silver bullet exactly, but it can really help.
Some people use a sitz bath. I prefer a hot water bottle (you can take a hot water bottle to bed). Just don't make it too hot. The warmth relaxes the sphincter, and eases the spasms. You can lie resting your buttocks on it, or tuck it between your legs.
Ibuprofen was the most effective painkiller for me. It took the edge off, but did not kill the pain completely. I'm guessing you are in the UK, so - neurofen for you. Be careful with paracetamol, it is really easy to OD on it, and it can be lethal. Stick to the recommended dose.
Really sorry to hear of your pain - been there - things are SO much better now. Hang in there! It's not for ever (though it feels like it).
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!
