If the fissure pain is gone why is my muscle still tight? :(

Hi everyone,

So I had LIS about two months ago and while the horrendous pain of the fissure went away I am still having some intense tightness down there during and after bowel movements and because of that I am unable to empty my bowels completely.

I can feel the tight ring of muscle around my anus especially after a bowel movement.

I told my CRS about it and he assured me he that his LIS cut was adequete, even though he cut the muscle to the length of the fissure and not to the dentate line. He flat out refused to put me under the knife again and warned me about incontinence if he did.

He said that he never had to do the surgery twice for anyone. I don't know what to do at this point. Should I go under the under knife with another surgeon? Should I wait? Should I self-dilate the area?

Can someone please give me any advice? I am very confused. If the fissure pain is gone why am I still tight down there?

I am considering having my rectum removed and getting a stoma to get this over with.

I would consider self dilation to help with this. There are many posts by others who can help you out with this. Start small and work your way upwards making sure to use a good lubricant. viewtopic.php?f=4&t=4261

Also, the tightness may be caused by a pelvic floor issue. If you can go see a pelvic physical therapist that could be very helpful.

9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy

I have been doing that for quite some time now and the problem is that it gets extremely tight after a bowel movement and thus extremely painful to insert anything in there.

I find it hard to grasp that it's a pelvic floor issue because this all started when I had my fissure.

I may talk with my surgeon and see if a permanent colostomy is an option.

Pelvic floor issues can begin when there is an injury and it can set the muscles off into a chain of dysfunction. Biofeedback would probably help you if you can find the right practitioner to work with. Don't lose hope!!

I know that. It's just difficult to grasp that a superficial, small fissure that has healed with LIS would cause that kind of dysfunction. Most if not all people return to normal after LIS.

Unfortunately biofeedback is not available where I live. I will not hesitate to go under the knife for a permanent colostomy for permanent relief. I just hope that my surgeon agrees. If it was offered to me now I will do it in a heartbeat.

This is no life.
