Okay quick update. I had an examination the other day, yup its back. There is a small, 2-3mm, fissure along the line of the wound. This was an examination by a different CRS so i'm now waiting for the consultant CRS to get back to me. I'm hoping that this is not a proper fissure but just a weakness of the healing wound. It has after all stopped bleeding thankfully so i'm just carrying on with the warm salty bath, movical, good diet and manuka honey.

Hi
Stevepain referred you to my post about advancement flap surgery. There seems very little about this operation on the net.
As you saw from my post I had a few set backs along the way but now I am more or less fine (no fissure).
I had my operation last june. The skin around my anus is still a bit sensitive so I wash with aquaeous cream each day and use sudacrem. I have a little soreness but nothing worth talking about really.
I am still taking one sachet of movicol per day but I think this is more due to habit or for comfort almost rather than a necessity.
I wish you a full and swift recovery.

Hi All,
Thought I'd got this little critter licked (not literally!), sadly I was wrong. The last re-tear seemed to heal and I was doing well until the end of this last week. Its torn again. No reason this time at all. I have been taking movicol every day, eating healthily etc but its gone again. I think the skin down there is just weak and maybe I have to accept that it will occur every once in a while. I have another appointment due early May that was organised from my last CRS visit which is just as well.
It is good to know that you are reasonably recovered Elfkin. Did you have any re-tearing occur after the surgery? This is my second time now and it is very tedious (understatement).
Has anyone else experienced re-tearing and recovered from these 'minor' tears? I hope so.

I'm really sorry to hear about the re-tear. About a month after surgery I did have blood on the toilet paper after a BM. But I had none of the pain associated with a fissure and it healed (in terms of the bleeding stopping) in a couple of days.
I did have uncomfortable BMs for several months after the operation but again once the BM was over no lasting pain (unlike when I had a fissure).
Are you experiencing the same level of pain as you were before the operation? Or just bleeding?

Thanks. The pain isn't as bad as before but this 2nd re-tear is worse than the last time it re-tore. Its bleeding each BM and a little afterwards with the pain lasting about 10 mins.
Did you do anything in particular to heal again or just time? This is now about 5.5 months post the operation. Image

I upped my movicol to two sachets a day (from one) and i also started using rectogesic (GTN) ointment again. From memory I did this for a couple of weeks and then I was more or less ok.
The bleeding stopped but I had low level pain for quite a while. I am 10 months on now and I still have a bit of soreness but nothing serious.
At 4 months I was still having painful BMs but nothing like my fissure probably 10-20% of the pain of a fissure but wihout the hours of agony afterwards.
Hopefully you are having a setback on the road to full recovery. I wish you well :)

Thanks again. Its good to hear that you went through a similar scenario and are now all but recovered Image . I suppose we are all slightly different and therefore no guarantee can be given but it does mean that theres a chance that I'm not necessarily going back downhill.
Today is a little better, not so much bleeding and less pain at the BM/post BM times of the day. So I'm hoping setback only as you say.

I thought I would update as anyone reading this for information would feel this is an unfinished tale. Since my last post I have seen my CRS several times for examinations and also another specialist to monitor muscle pressure of the sphincter. They do this with a probe and isn't very funny. The result of this was too much pressure and LIS was agreed upon. Well cutting a long story short yesterday I went for LIS but was surprised to find myself coming round from my 'op' only 15-20 mins later.
Good news: My CRS told me he'd performed a sigmoidoscopy (a full on examination) and the fissure was no longer evident so the LIS op was not required. Hopefully I can continue the regime I have and will not have a re-tear, if so its sorted Image . I have thought I've been at this point before but having done so I intend to remain very careful and very slowly head to a normal existence.

For those who are reading this and are perhaps being offered advancement flap I thought a quick update again. No re-tear has occurred and I continue to slowly get back to 'normal'. I am not yet at the sport activity stage of confidence yet but I can certainly do more than I have for a long time. I would say if you are offered this surgery ask if there is any other options first to be considered as it's a fairly big slice into your backside that takes awhile to recover from but if it's the only option left then go for it. I'd rather go through the last two years than have an AF for years/life. :)
