Anterior & Posterior Fissures

New to the forum. Doesn’t seem like there’s too many active posts anymore but hoping I can get some help.

Quick history.
Gave vaginal birth 6 weeks ago. I have always had a little skin tag on my anterior portion (never bothered me before). Had hemorrhoids throughout pregnancy that never bothered me. While on postpartum leave I was sitting much more (nursing) not working (less active) and probably eating pretty bad.
2 weeks ago I remember having an episode of constipation where I had to push pretty hard and felt a mild pain/tear. As the days progressed it became extreme 10/10 throbbing pain for 6+ hours after every bowl movement. I finally went to the ER & was referred to a general surgeon. She did an exam and said she could see a 1cm posterior midline fissure and didn’t see an anterior one. (However over the last week I have gotten to know my bum a lot more and I definitely have an anterior one under the skin tags).

I’m doing sitz bath after BM with peri bottle and if I can other times a day, miralax, psyllium husk, small dinner at night, (beans, mashed food, guacamole, little bit of chicken), ditelazam with lidocaine applied with the doserite applicatior, and finger method (V) while having BM. Also trying to do yoga and pray constantly.
**I have had some relief in my posterior with V method but the anterior one is definitely still painful and bright blood is always on my fingers there after the V.

Looking to see if anybody can shed light on

1. Having both anterior and posterior and what you did to recover
2. Do you think the skin tag on the anterior means it’s already chronic and not going to ever heal? (definitely don’t want to do surgery and can’t do Botox if nursing)
3. Is 1cm large? Seems large

**update I have 3 anterior fissures in addition to the posterior

I would recommend you go see a specialist colorectal surgeon, versus a general surgeon. Also check with your obgyn if your estrogen levels are low and need to be supplemented. They can be reduced in breastfeeding women.

I don't think the skin tag means it will never heal, it is just the body's response to an injury. I'm not sure if 1cm is large or not.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy
