hello my kin in butt pain

I'm 22, I was diagnosed with IBS about 2 years ago, I've had hemorrhoids my whole life and had a haemorrhoidectomy after a blood clot had formed in some of them February 2023.

Around 10 weeks ago, I was on a flight back home when all of sudden I got a sharp, deep and unsettling pain, we landed and I had some of the worst diarrhea I had for a while. Got covid. The pain never left and my butt was feeling pretty sore so I called my doctor but he brushed it off, until about 6 or 7 weeks ago when I had a *presumably* lung infection that sent me to the hospital. Within less than a week the butt pain grew too much and I demanded to see him. It was a severe rectal fissure.

The moment the doctor pried open my cheeks I screamed in agony, a banshee's screech that alarmed him. He called in another doctor to confirm and it was in fact a severe rectal fissure. They PROMISED me around 6 weeks recovery time, I called them again about a week ago and they said 6-8 but that to me seems doubtful since I can still fricking feel how bad it is.

It's currently 6am, I'm half drunk after my sister's birthday, super tired but I can't sleep because my butt hurts like hell. Feels like a smoldering iron got shoved in my hole sideways. I can't find a comfortable position cause of my scoliosis and butt, wtf do I do levitate? I've had 3 VIOLENT BM since midnight and I'm exhausted. I should probably add that I'm severely lactose intolerant and I forgot to take my lactase pills before eating pizza and whipping cream

Sorry that you're in pain. I think you need to do some deep breathing exercises - there are some listed here:

When you feel more relaxed, try to sleep. Then contact your CRS and explain your situation to them.

I managed to pass out around 7, it's 2 in the afternoon now. What's a CRS?

Glad you got some sleep. A CRS is a colorectal surgeon.

I called him about 2 hours ago, he said when I get back from my trip (it's a few days starting at the end of June and ending at the start of July) to contact him to have another visit and see how it's healing. I'm again hoping for the best but I know there's a big chance I'll be disappointed (in the sense that this dumb fissure won't closeup any time soon), just like when he told me that the healing time would be 6 weeks but it's quite clear by reading everyone's experiences that it's definitely not 6 weeks and I myself I'm past 6 weeks

Well, you could always get a second opinion from another CRS. But yes, fissures take time to heal, and sometimes they don't ever heal completely at all. Reading around on the board is one of the best ways to listen to other users' experiences, and I encourage you to continue with that.

Hi there - I know how awful this is. You might want to start thinking about self healing - which starts w diet and managing the IBS (which I know you know!) My GI specialist made a point to say for severe fissures (which I have) the number one priority is to keep BMs soft - but not too soft. It’s so hard. can you get a prescription for GTN ointment from your doc? That’s helped me heal somewhat and helps w the pain! I also heard lidocaine helped take the edge off. Feeling for you!

Hi there. Fissures like yours need a great deal of care in order to heal up. That means pressure reducing ointments, sitz baths, an easy to digest diet, a mild laxative like miralax and stress reduction (easier said than done). 6 weeks is not a magical number, it can take longer than that as long as you take great care. Check out the diaries section for stories on how other forum members have healed. Take care and wish you speedy healing.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy

what are GTN ointments? and what are sitz baths?

GTN ointment is nitroglycerin - so basically what is happening w your fissure is you’re not getting blood flow to the area you need to heal. Lots of stuff in these forums about that. The ointment helps relax the blood vessels and bring more blood there so healing helps. And ideally stops the spasms which is the pain you’re feeling! Sitz baths are basically things you put over your toilet and fill w warm water and that helps calm the area too altho for me it never really helps :/
