First it was the piles then the fissures now back to piles!!

I hate the piles (hemorroids). It all started with constipation and piles 4 months ago. The red bump came out of my anus before and it was painful, after which I developed fissures after a few days. Then the pile became silent while the fissures caused me to suffer for months. Now after managing my constipation and all the softening diets my 3 fissures had turned quiet and is IMO, healing that remaining 10-20%.
However last night I was shocked to see that the Piles came out of my anus! And it irritates my fissures causin it to be rather painful!! Argg... Really dun get it. I wasn't quite straining to pass stools for days and it was quite easy and smooth bms, so why?? The only thots I have is sometimes I do strain to keep my farts i cos prune juice gave me some gas and these 2 days I have been standing and walking a lot, is that y the piles came out? This morning, it has shrunk and went back In, but... The fissures has some pain already... Sigh!!
Pls...can anyone advice me??

Aww Dwarf! That sucks! I'm so sorry you are struggling! Have you seen a Colorectal Surgeron (CRS)? That's the specialist you really need to see, especially if you've had fissures, even off and on, for four months. At this point, I would consider the fissures chronic and medical intervention is needed now. A few boardies here had hemmies at the same time they had fissures, and some even had their hemmies banded at the same time they got an LIS or botox for the fissures, others got their hemmies banded after their fissure surgeries, and banding the hemmies might even be possible while using ointments for fissures. Quite a number of boardies also had hemorroidectomies at the same time they got an LIS. So hemmies and fissures together are not uncommon, and they can be treated at the same time. If your hemmies are prolapsing - coming out of the butt - then I'd say that also requires medical attention. I haven't had prolapsing hemmies myself, but I did a lot of research on them when I was suffering with my fissure. If I'm not mistaken 2nd, and especially 3rd and 4th degree hemorrhoids prolapse, and the sooner you get a 2nd degree hemmie before it progresses to 3rd or 4th degree, the better. Perhaps others can chime in. I would say get to a good CRS asap, especially since it sounds like the hemmies are aggravating the fissures.

Hey Dwarf,
Sorry to hear your having some trouble :(
Have you tried warm baths, maybe ice? It seems once things go bad in that area, it's almost impossible to get back to normal again Image Right when we think we're moving forward, out of no where something happens again....
Has you CRS said anything about your piles? Did he/she think you may need surgical treatment? Hopefully the swelling will go down soon! It has to be making the fissure feel worse.
Hopefully someone who has experience will come along with some advice...

hi Dawn and hope, thanks for replies!
Spoken to my CRS this morning on the phone, he mentioned that everyone has piles and it's really no cause for worry unless it starts to bleed (which i am not) and/or hurt. He said if it went back into my anus, then it has shrunk quite a lot already and give it a few more days, it should be much better. I asked him why did the hemmie prolapse though i wasn't straining, he mentioned it can be due to too much pressure on the anal region since i mentioned to him i did stand / walk a lot these 2 days, or it can just happened out of the blue (OMG!!). But he did mentioned that once a hemmie has prolapsed, the muscles will become looser, and will prolapse more often! (SIGH!)
I told him that i did have taken the medicine (to protect the blood veins) for the hemmies before, but it doesn't seems to shrink anything as i can still see the tip of the internal piles. Soo... all in all, he asked me to relax for a few days, continue my fissures diet as usual and monitor. If it gets worse, i'll have to see him....
He did mention colonscopy to eliminate other causes too (eg. tumors etc), but i really dun think i wanna RISK tearing my 90% healed fissures!!
Anyone knows what are the supplements i can take for hemmies?

oh ya forgot to mention... Even holding in your farts can have some pain/discomfort. Anyone who has fissures and/or hemmies like me??

Rutin is the supplement for piles, although I haven't tried it personally.
I think piles are very common with a fissure - I had one that ruptured which caused the fissure!
Yes, definitely ice to shrink it and lessen the pain. An ice cube in a little sandwich bag works great!

i also have this problem of a external hemm after i had a fissure heal. i have been using ice on the external hemm to shrink it and went to my crs and she gave me some new creme to use. i am feeling some better after the creme. the crs wants me to come back in a month to see if i am still having problems with the hemm and if so she will take the hemm off with outpatient surgery. i am not sure how the surgery is done.

Gosh Jenny, are they sure that's not a skin tag that's formed from your fissure...hmm?

I couldn't even pass gas when I had my fissure without pain. Some days were better than others but none were good. I would seriously consider that colonoscopy. Hems prolapsing doesn't sound good. Maybe they can be banded and that may even help your fissure heal without all the extra swelling Image Many folks on here said the colonoscopy wasn't bad at all and they had active fissures. I understand your fear though. I was the same and refused to get one when I had my fissure.

dawn, no i had this hemm before i even had the fissure but no trouble with it until now after the fissure has healed. always something..
