Terrible Pain can anyone help with suggestions please :(

Hi I am new to the forum and I am in terrible pain. I have had the fissure for around 7 weeks now at first I thouht it was hem's because when I was oin to the toilet I was bleeding alot then the pain started about a week later :( it is like passing glass and the stabbing throbbing and burning is getting me down.I went to see the specialist 2 weeks ago where she did a sigmoidoscopy and even then when she inserted her finger I nearly jumped off the bed she told me I had a fissure and skin tag plus Hems,the bowel prep I had done did not work properly in clearing the bowel completely so I have to go back in a few weeks so she can do it again :( The doc prescribed me Dilatezem twice daily and bran flakes in the morning but it still hurts like hell. I have not slept properly with the pain for days now and cry constantly at not only the thought of a bowel movement but during and with the pain afterwards it lasts all day Image day after day. Does anyone have any suggestions as to what I can try to ease the pain before I go out of my mind Image

hi sweet, so sorry to hear that u've been in pain for a while now...

if u are having hems, u may want to heal from within using vit c with bioflavonoids, rutin and hesperidin. This can help u to speed up the healing and also tone up your blood vessels which in turn helps ur hemms. Not sure if ur doc has prescribed daflon for your hems, it cld help u to a certain extend as well. U mentioned lots of bleeding, is it in the toilet bowl / toilet paper? If it's mainly in the toilet bowl, then it's most probably frm hems. If toilet paper, fissures normally has some blood on them, not a lot like hems.
Also, if u can, try the horse chestnut cream which tones up blood vessels and improves circulation too.

Thanks for the reply Dwarfs8 :) I am struggling on what foods to eat as I am terrified of when I need to go to the toilet Image I read that success story earlier on and its good to read that people do it and overcome this terrible pain. Do you know if packet soups are ok to eat like Chicken Noodle? I have loooked and it contains cornflour and it worried me also tuna I am going out of of mind here with worry Image I am also diabetis type 2 with an overactive thyroid to which I take medicenes for I am so confused Image

hi sweet,i'll try to advise u best to my knowledge, but i feel that different foods react differently with individuals, so anything unsure, u shld first take a small amount and see if your next bowel movements are alright before confirming them "safe". Tuna contains magnesium (attracts water in the gut and softens stools) and is probably the only meat i can take with ease, along with fish (contains omega 3 fatty acids gd for digestion too) and chicken. Avoid beef and pork for now as they are harder to digest.
If u are eating pasta, make sure u eat wholemeal ones, as they have high fiber. Skip rice for now and refined white flour breads. Take wholemeal or multi-grain breads. If you can, take half glass of prune juice (u have to try out the different amt urself to reach ur desired state of stools) with ur meals once a day or more if u need, DO NOT TAKE TOO MUCH in a day, as it can lead to diarrhea. Cornflour is ok. I use quite a bit in chinese cooking and that's fine. Soups are ok as they are liquid anyway.
Try to eat some vegetables and/or fruits with every of ur meal and aim to have at least 1 BM everyday. U have to develop a habit of having the BM at a certain time of the day, eg. after a meal, or early in the morning. This ensures that ur stools do not stay in you for too long and turn hard.
REMEMBER to drink lots of water during the day. This helps to keep the stools soft. REMEMBER the more fiber u take, the MORE WATER u need to drink, else it will be hard too.
Flaxseeds, barley, oats all these are natural to keep the stools soft. Some fruits to me can make my stools hard as well, such as banana and papaya. Apples and pear contains pectin that soften stools. For me, grapes are the best.
IF all else fails, take miralex for the TIME BEING. I heard people on this board takes this with good results.

Thanks for that Dwarfs8 I love Tuna and I am going to have some now as I am starving the for tea I am having roast chicken no skin with ve ..cabbage and mixed veg I am also drinking lots of water and cranberry juice. I drink green tea all the time but I have put that on hold afraid it could be that thats hardening my stools Image Can I ask you if thats ok how long have you had your fissure for?

i don't dare to take much green tea too unless i'm having jap food. Tea has caffeine and thus dehydrating, so better skip it for now.
i had my fissure for 5 months, before it starts to calm down. During that 5 months i hav plenty of ups and downs, heal and retears. It only start to calm down after i have started my sitz baths twice daily and vitamin c supplements. Also i apply aloe vera everyday. So dun be discouraged! we all faced the same healing curves here!
remember to keep a food diary so u know what sort of foods can soften, what can't for u.

Hi there, Really sorry your suffering! It must be harder for you being a type II Diabetic too :(
There is good news though! Read around on the forum as there is alot of very helpful advice on here.
Personally, I found the diltiazem worked great for the first 5 months and then I ditched it for Vaseline. The vaseline helps immensely if applied b4 and after a bm. Also, magnesium supplements have stopped the ache and spasms down there. Main best friend though is the stool softener Movicol/ Miralax. Then u won't re-tear or have that terrible terrible pain passing stools ouch!!!!
For foods to avoid, well agree with others who already wrote on here. Especially stay away from beef, but minced meat in spag bol fine lol!!! It is trial and error. Good luck!!

sweet, did ur CRS give u lidocaine jelly? tat helps a lot too. When i first had fissure, i went to a gp who INSISTed i do not have fissure, and said i have hemms instead. Only gave me a hemm cream which caused my fissure worse. AND then i was in terrible pain like u for a week, i wanted to kill myself (thots only). Eventually i went to specialist, one look he knows its anal fissure and gave me the GTN and jelly. Tat takes away a lot of the pain. so make sure u have the lidocaine jelly before bm.

My specialist just gave me Dilatazem onitment not a jelly been using it for 3 days now and not feeling much difference. I can only use it twice a day not after each bowel movement and that really sucks since I go to the toilet at least 3 times a day so I am taking Ibuprofen for the pain but it really does not touch it :( I can't concentrate on anything when it gets this bad and feel really miserable and thats not like me Image got my cabbage and broccoli carrots and cauliflour with some chicken for tea so lets keep our fingers crossed oh yes is gravy ok to have? Thanks again for all the advice i really do appreicate ityou guys are wonderful Image

of cos gravy is fine...haha... anyway, u shld ask ur specialist next time for the lidocaine jelly. It helps to numb and lubricate ur anus before BM. For now, u can try use vaseline / aloe vera / vit e oil as a lubricant before BMs, trust me, it helps A LOT to have the stools slide out without much efforts, tat in turns helps to calm urself down while doing it, and thus reducing the pain.
Also, do CHEW ur food v v well before u swollow. This helps in digestion and what comes out after that. Ibuprofen, i'm not sure if it will give constipation, so for the time being , u probably wan to take miralex as well.
I'm quite sure u can use dilt ointment after each bowel movement, i don't think that's a prob. I use the same for my GTN cream, no prob at all. BUT i don't think it helps as much as aloe vera for me, cos GTN do not have healing properties, but aloe vera has.
