Does anyone feel there pain internally?

I have been having excruciating pain during bm, even if my stools are just a little bit firm. There was also blood in the middle of the stool but none on the toilet paper. Does this indicate internal fissures? Image

Sounds like it, to me.
I haven't really had blood, but the pain has been horrible (at its peak).

I would go back to your CRS. You should see someone who can make sure it's OK with a scope.
