What else can I do and shoud I be worried now?

Hey all.
Just discovered this forum, and it sure seems great!
First, I would like to tell you my story and then Im hoping to get some input on what I should do. So here it goes.
Im a male in my early 20s. As long as I can remember, Iv had problems with my stomach and, yeah well, my ass. Problems like stomach ache, diarrea, constipation, bleeding when going to the bathroom and things like that. All of these problems were on/off problems. But about 4 months ago, it started to get more serious. It started to hurt every time I had BM and sometimes it would bleed, only a very little that is, with some red spots on the toilet paper but never in the toilet. The pain was like craping something sharp. At first I thought I had hemorrhoids but after reading more and more on the subjeckt I then realised that it must be an anal fissure. At first I dident take it very serious as I always have problems in that area and figured it would go away on its own. About a month ago, the pain started to get worse and I realised that this needed some treatment. I then started to eat a lot of fibres and drinking lots of water. Iv always done a lot of exercise, though the anal fissure accured in a period of me being lazy due to depression, amoung other things. This helped a little as my stool got more soft and easy to pass but it dident help the healing of my fissure at all.
Most times it only hurt then passing stool, afterwards I would not feel it. Sometimes though, it would feel very tight (spasm?) and then I would use a little cream (made for hemorrhoids) to take the edge off.
Well, a week ago I realised that my treatment had failed and that I needed to take new things into use. I started to use hydrocortisone cream 1 %. Dont know if you got that stuff in the states? I also began taking a fibre supplement. Uptil now, no relief.
Today, while making my daily morning visit to the bathroom, I got scared out of my freaking mind. The pain was as it always is, annoying and painfull. But this time there was alot of blood! It was in the toilet bowl, on the stool, on the paper and not in those small amounts that im used to!
I called my doctor right after and got an apointment tommorow. This will be the first time that I involve him as I thought that I could solve this by my self.
So, what I would like for you guys to do is to tell me if that amount of blood is normal for some of you or if it sounds like I got reason to be scared (cause I really am) and most importent of all, what else is there for me to do?
Im thinking about trying the Nitro 0.2 % cream but it seems to have some side effects.

Hi there and welcome!  
We all freak out at the first sign of blood, but find out in the end - even if it drips all over the place - that it's no big deal if it's bright red. 
Still, it's just as well to see your doctor. 
Nitro does not have the same responses in everyone, but a lot of people do get headaches at first, which taper off after couple of weeks. 
Hydrocortisone is not the greatest for fissures; Nitro, Diltiazem or Nifedipine are. Rather see what your doctor says tomorrow, and then come back and check up on the docs suggestion. Believe me, whatever the doc suggests, someone here has tried it. The search facility up on the right top is very good.

Update:
Today, before going to the doc, I went to the bathroom. This time was much more scary than yesterday! It made yesterdays BM seem like a vacation. This time, blood everywhere! Never seen anything like it and I got pretty nausea from the looks of it. The pain though, was not worse than always (which is bad enough) and this seems a little odd to me as the bleeding has never been anywhere close to what it is now.
A little later, it was time to see my doc. I told him about my symptoms and that I was pretty sure it was an anal fissure. He took a look and quickly confirmed that I indeed have an anal fissure, about 10 mm long and 2-3 mm deep (does this sound normal, bad, not bad at all or what?)
We went on to talk about my diet, but all his advices I have already been following for the last mounth so no help there.
He went on to say that I should try Movicol as it is very importent to keep the stool soft, which I think was the problem today as the stool was big, hard and I had to press to get it out. I just purchased the item and crossing my fingers that this can help me.
He also got me a prescription on some cream, dont know which one yet as I can first get my hands on it around 3 pm but Ill get back with the name on it then I get my hands on it.

I just started out with Movicol. It works but couple it with plenty of water the first couple of days using it!

I drink like 3 liters of water daily so thats a given in my case :)
Okay, so I just went to pick up the cream and Im not really that happy about it as its not something that Iv read anything about in the treatment of anal fissures. The product is called "Doloproct" and its containing the following:
1 gram of cream =
Fluocortolon-21-pivalat 1 mg
Lidocainhydroklorid 20 mg
Bensylalcohol 5 mg
Is this worth a shot or is it worthless?

Hi there!
As far as I can see,it is for the treatment of hemorrhoids. I think there is something similar that my GP prescribed for me by the name of Proctosedyl. It did nothing for my fissure so stopped taking it and asked my GP to refer me to a CRS.  GPs are not the greatest for Anal Fissures. The sooner you see a specialist the better, as having had yours for more than 4 months, it's on the way to being chronic, if it's not already. Have a read at posts written by Davo for a good idea of how these things progress (check the Memberlist, then look for Davo, then look on his profile - on the right - for posts that he started). Hope this helps.

Thanks for your answer and the link, Savaici.
In the note that came with the cream it says that its for hemorrhoids and anal fissure BUT it also says that 1-10 % gets the side effect "thinner skin" so I really dont want to use this. Ill just go back to my GP next week (his on vacation this week) and ask him for eighter nitro, Diltiazem or Nifedipine. If the condition gets worse then Ill ask to see a CRS (at this point, my GP thinks "he" can fix it, but he also want to use the nitro as the next step so at this point I dont see why I need a CRS - feel free to tell me why)
I just took a look and some of Davos post, and even though it seems to me that he got out allright after doing LIS (right?) it still made me a little depressed that he could be doing so good on the nitro and then have a re-tear - this fissure thing truly sucks!
