How to tell if it is caused by spasms

Hi all I am new here so sorry if this is a dumb question.
I have severe sharp pain during bowel movements, and sometimes sharp pain without passing a bowel...but mostly just during passing. It's excrutiating and I have blood on the tissue when I wipe.
I saw CRS yesterday who said it's due to spasms and that my rectum is very very tight. Recommended that LIS surgery. But I don't know, it confuses me...it doesn't feel like a spasm to me. Just a sharp sharp pain like I am being poked with a sharp piece of glass or a thumbtack. Is that a spasm?

Hi there xanderla30,
I would say that your CRS is right, that the spasms are what are giving you the pain. The trouble with AFs is that when you have had them for a bit, the involuntary muscle has taught itself to do something different, to protect itself, so to speak. But in the process of its new route, we go through pain. You seem to have the very symptom of an anal fissure: as though you are being poked with a sharp piece of glass, and that is the spasm.
