I'm sure there is a lot of variability on this one! I have a pretty darn big AF (2 cm by 1 to 2 cm), at the site of old scar tissue. I really thought I might have developed another fissure because my pain seemed 360-degrees when I had a BM of any substance. But my PA (CRS clinic) just ruled that out, it's the main fissure causing symptoms (most likely). He didn't get a great look, I don't think. I have teen/young adult children, so I'll let you guess my age! My advice, if you don't feel comfortable with opinion #1, go get another opinion (or so). This is really a terrible problem to have and I think you should feel confident in whatever path you ultimately choose. In the meantime, work on getting your stools softer and easier to pass!
