Please help, chronic, deep anal ache, is this a fissure?

Hi there, this is an embarrassing one, even by the standards of this forum, but any help would be greatly appreciated!

My boyfriend and I are into both vaginal and anal sex and three months ago he went deeper in me than anyone has before with a ridged sex toy, I think he went past the rectum! When he pulled out, it hurt, and I've been in chronic discomfort ever since.

My main question is, is this an anal fissure? It might be slightly different. Firstly, unlike many fissures, the discomfort is deep inside me, perhaps 4-5 inches inside. And I experience a chronic aching sensation, rather than a stinging sensation, that feels like it affects an area of about an inch.

Also, I've never experienced any blood in the stool, although I gather not all fissures present with this. When I go to the bathroom I experience a combination of aching and just slight stinging sensations, but it could be worse. Afterwards, sometimes I experience more aching than usual for a little bit.

The discomfort level of the ache is sometimes better, sometimes worse, but really hasn't improved overall in the three months since it happened. I experience it 24/7.

I've also had 5-6 more serious flair ups when for maybe 30 minutes to an hour, I experience a lot more aching in the area. This only happened once after going to the bathroom, all the other times were all when in bed, trying to sleep.

Treatment wise, I'm trying to drink a bit more water, taking fibre supplements twice a day, and having daily hot Epsom salt baths. The hot baths help a lot when in them, but pain soon builds again.

I also bought some Dr Greengrass, that I was thinking of trying to work up there deep, but I'm afraid of making it worse. I'm also going to try stool softeners for a couple of weeks at least.

So does this sound like a fissure? Could a deep anal sex injury like this be anything else but a fissure? Because all the information online suggested this is what it would probably be. But I think the pain is coming from the area where the rectum meets the colon.

What else could I try?

Hi Sufferer. Sorry to hear you are in such pain. Unfortunately, I think the only way you will get some answers is to go get checked out by your Doctor, ultimately a colorectal surgeon who can look inside and see if there is any damage. It could be muscle related or it could be an injury. Has it been with way for 3 months now?

Thanks for getting back to me.

Well I thought perhaps aside from a deep fissure, it could be an injury to the muscle that connects the rectum with the start of the colon.

You're right, I may have to go see a doctor, but it's pretty damn hard to work up the courage. I'd rather give it more time and hope that the pain starts to get better. Yes, it's been three months and the pain has been pretty steady in that time.

My biggest fear is that it could be some kind of injury that I should've got checked out right away, and by leaving it, I'm risking permanent damage. But it doesn't seem to be getting any worse. If it does, I'll really have no choice but to go see my doctor.

Go see the Doctor - I waited awhile thinking my fissure would heal, and it just would not...I did let him know I wanted to avoid surgery if at all possible, and he prescribed an ointment that has done wonders, I felt the difference instantly. Unfortunately I think I ripped in partially open again this week when I'm scheduled for my follow up visit, so we shall see.

In any case, I also suffered a throbbing ache that often lasted for hours and was unbearable to the point I could not work on some days, and I have an office job. My biggest regret was waiting to go to the Doctor, as I could have relieved my pain so much earlier.

As for being embarrassed, trust me the CRS has seen it all and then some - so tell him/her the truth, and get started on whatever your path is now instead of putting it off and making it worse.

Yes, please do not put off a dr visit because of embarrassment. I don't think you can be a CRS if you are easily embarrassed or shocked. :lol:

It doesn't really sound like a fissure, because for most people I think fissure pain comes and goes somewhat - agonizing BMs and spasms that last hours, but generally some relief for a few hours (until the next BM). But you will not know for sure unless you get checked out. And three months of pain that's not improving - you need to get that looked at.

I hope you can get this resolved. It sounds really miserable, and I don't think you should live with it a moment longer than you have to.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

OK thanks for the feedback guys. Here in the UK you need to go see a General Practitioner (GP), and they refer you to whatever specialists you need, so that's the route I'll have to take.

Yep, I've read enough about fissures online now to know that it could be something else. 1. The pain is an ache, not a sting. 2. Going to the bathroom is slightly more uncomfortable, but not the nightmare some of you are describing. 3. I'm suffering 24/7 steady discomfort, it doesn't come and go. 4. No blood in the stool. 5. No spasms.

Frankly, I'd rather it was a fissure, at least then I'd know what I'm dealing with and there are many treatments available, but I suppose the 5 points I've summarized above do not rule out a fissure for sure, it still stings slightly when I go to the bathroom, and they come in many shapes and sizes, and I realise some of you are reporting an ache, not just a stinging sensation.

Anyway, thanks for confirming what I thought, I need to get referred to a proctologist or CRS.

Reading some of these posts on this forum, I can't believe what some of you have been through, best of luck with it all! In my research about this I read a lot of positive reviews for 'Dr Wheatgrass' as a treatment, in case some of you haven't tried that one.

Maybe I can call the CRS department directly and bypass having to see the General Practitioner.

Depending on where you are in the UK, there is a great CRS in London - Mr. Windsor. A few of our members have seen him.

Can't speak for UK's system, the USA certainly has it's good points and bad points...I think the key is a good CRS. And while there are deeper fissures, generally you can actually feel it near the exit point with hardly any insertion, I certainly can with my finger. It also hurts to touch it, so those are the signs for me - besides being confirmed by the CRS. Yours is deep which means it may be something else...again there are deep fissures but I think they are more rare.

Please go get checked out but I agree with Simon... It might be a muscular problem which is something I've been dealing with for the past 10 months...:(

Thanks then. I've been referred to a CRS now. The doctor that referred me was totally unphased by it, and I'm so glad that I found a good GP I hadn't seen before. While I'm sure all of them have heard it all before, some GPs can really make you feel awkward and look slightly judgemental, while others like her just react as if I'd described any run of the mill issue like a persistent headache.

The pain varies quite a lot for no apparent reason. And is usually either better or worse for one day.

I'm really worried it's a muscular problem. Speaking of headaches, if it helps anyone here to offer any more advice to me, it feel a lot like headache pain. I've reached inside myself with a rubber glove and I can't reach anything that felt sore, but afterwards, the aching was worse that night.
