GTN including usage and personal update

Hi,
After my previous course of GTN I've managed to get through over a year with no major issue but things have never seemed quite right down there and felt close to retear a few times. I went to see my doctor about another issue recently and mentioned that my fissure issues were mostly settled at the moment but I often experienced pain trying to release gas and he quite rightly suggested another course of GTN to see if it will help finish off the healing process. Anyway, due to being very busy I delayed it a week or so and after going hiking (which I've been doing a lot of)I started experiencing pain so immediately started using the GTN. It didn't help at first despite the fissure not but being too bad but now seems to be settling again.

I have also contacted a private hospital and had my first appointment and it's worth noting that according to this doctor who is a specialist, he stated the GTN should not be pushed inside and only about a bead sized amount needs to be rubbed in externally to the opening - this is apparently enough as it gets absorbed and relaxes the muscle(s) and increases blood flow. And it's funny he mentioned this as I did once go a little too deep recently and actually it really didn't feel great all day, including horrible burning sensation from what felt like further inside the anus.

I am now having going to be having a blood test, stool sample test and not sure what they call it but an internal inspection (tube inserted) to check for any bowl/lower intestine problems, including crohns/colitis, then if those all fine will be a decision on trying botox (60% chance of success) or just jumping to LIS which should definitely help. As I've now had issues since 2008 I really just want this sorted and put to the back of my mind as it's interfering with relationships (don't want to start them) and due to be super strict with my diet my weight is now about 62kgs (<10 stone) and I'm a 6 footer (so just about in the normal zone in regards to BMI). Overall I am feeling very good at the moment so not too worried about gaining weight and will now always maintain a very healthy diet (wasn't too bad before to be honest) but I do not want to lose any more weight.

For anyone in the UK without private health insurance it's worth noting that you can combine private with NHS, ie, I've paid to see a specialist but blood and stool tests will be done and paid for through the NHS and I'll pay for internal inspection probably and then definitely the LIS or botox to get that done ASAP assuming there's no other issue causing the fissures.
