What causes high resting anal sphincter pressure?

I've been reading the book a headache in the pelvis (Wise & Anderson) and found this passage that could explain this question.

"People who have had some kind of injury and pain that has caused the pelvic muscles to reflexively tighten against the pain, the pelvic muscles don't relax well after contraction. They stay in a tight state or spasm and when the muscles don't relax, all kinds of weird symptoms occur....The state of chronic constriction creates pain referring trigger points, spasm and chronic hypertonicity, reduced blood flow, and an inhospitable environment for the nerves, blood vessels, and structures throughout the pelvic basin".

Alyssa, your quotation makes such good sense to me.
I've asked my colorectal nurse practitioner a couple of times about relaxing the internal anal sphincter using biofeedback. She doesn't know if it's ever done. I feel sure it could be, but haven't been able to find out more.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

My understanding about tension in the internal sphincter is that it is either caused by injury in the area and/or stress. Deep breathing and relaxation exercises are supposed to help.

In your book, Alyssa, Dr. Wise goes on to explain how he got his resting pressure to zero using his breathing/relaxation technique. I wish I could say that I have had success with that.

Otherwise, the ointments (nitroglycerine, nifedipine, diltiazem) would lower IS resting pressure by up to 30%. But of course this only works on the internal sphincter, however, it apparently contributes about 70-80% of the resting pressure in the anal canal...

Thanks for recommending this book... I am learning a lot about this Pelvic floor dysfunction... How did he get his resting pressure to be zero... I suppose I'll read that in the book.

You are very welcome. I thought the book was great.

They have a special breathing technique they teach you in the book. I really got a lot out of that book myself. FYI, if it comes to it for you they actually run clinics in your state. They are expensive but they teach you how to treat your own pelvic floor dysfunction. I have considered coming from Canada to check them out.
