3 year old son with chronic severe anal fissure....ideas!?!

New here as of right now...stumbled here and glad I did. My son was born with spina bifida and therefore is paralyzed waist down and has no anal tone at all, which makes his fissure unique. It's likely caused from prolapse rather than spasm or constipation. His fissure has been growing worse over these months and nothing we have tried has done anything. Even doctors are perplexed. :( I am so frustrated. The one thing I have not tried was Manuka honey. Thoughts?

valleygirl,
I'm so sorry to hear about your son and his problems with fissure. Have you been able to get him seen by an experienced colorectal surgeon who might be able to shed some better light on options for treating his fissure?
Manuka honey can't hurt and might help. A few people here have had some good results with it. But in your son's case, which sounds much more complex it seems like there has to be a specialist out there somewhere who can shed some light on this problem and help with a solution. Are his bowel movements normal, soft, and easy for him to pass? Has any doctor suggested anything to you about treating the prolapse? Has he had any kind of testing done like manometry or something similar? How long has your son had this fissure?
Welcome to the board. There are a lot of really amazing and supportive people here and as you browse around you might find some tips that could help. It must be so heart wrenching and frustrating for you and so frightening for your son to be in so much pain. I hope there are some answers forthcoming for both of you very soon.

LUCKILY he has no sensation in the area, thank GOD, but I am concerned if it continues to grow worse what might happen in terms of what he feels. Thanks for your sympathies. He does have a colorectal surgeon who felt it not worth repairing seeing as it would likely just happen again or not heal properly because of his other issues. He put him on a bowel program where we do nightly enemas to help him empty the colon where he can stay clean during the day but that irritated it more as well. His bowel movements otherwise are mostly soft but since he lacks sensation does not know when he needs to go and often just a bit will come out and then later more and then later more. So it's constantly being bothered down there. :( He has had a contrast enema down to check his internal anatomy and all is well. No other tests have been suggested. I am waiting for a call back to hear his plan and have another call in to a center through Cincy Children's which is one of the best (I am in TX so not convenient at all though). Hoping to heal on our own.

Geez, what a nightmare. The only good news being that he doesn't have pain. It would probably be worth a second opinion from another specialist just to make sure they are on the same page. You never know what another doctor's experience might be able to add to the mix. In the meantime, the best you can do is supportive care to keep the area as clean as possible, which is what youre already trying to do. Do you put anything on the fissure to protect it from stools? Zinc oxide or Vaseline or maybe some Calmoseptine cream to keep a barrier between the fissure and the feces? I'm just thinking out loud, but it could help to minimize the trauma to have a good thick barrier cream to protect it. Maybe pass that by his doctor if you're not already doing something like that?

I put stomacare powder and then sensicare cream over it. Sometimes I will use a barrier spray as well but don't notice a difference when I do or don't.

I was thinking, if he paralized, then he probably sitting most of the time, which can cause luck of blood circulation in that area; so what if you provide him with a good memory foam doughnat pillow. Any warming device will need close monitoring so it won't cause a burn. I also thought about substituting his enema with glycerin suppository( I've seen them in kids size and even considered buying for myself ), it may have the same effect plus lubrication. Is his fissure internal or external?
I'm not autorized ofcourse to give any medical advise, especially in such unfamiliar situation, I just share my thoughts. Image

That's a good thought on the donut...but he is mobile in the house scooting on his butt all over the place. In his wheelchair when we are out the donut might help a bit but that time is not anything at length right now. We have stopped doing the enemas since it seemed with the lubrication we needed and what not the fissure was getting worse. Suppositories are very difficult since he has no anal tone at all. His fissure is external. Thanks for the thoughts.

If you find small doughnat ( or you can make one from cheap foam pilow ) you can put it in his pants on top of the diaper. The idea is also to reduce the pressure, because when he scoots on his butt he might rip the fissure, and he doesn't feel it. When we have fissure, we can't sit at all due to the pain.
The fact that he is mobile is great!
